Guthrie Healthcare is Hiring a LPN Office Coordinator (Home Health) - Full Time (Up to a $15,000 sign on bonus) Near Oswego, NY

Position Summary:

Coordination and conduction of clerical functions for the Hospice office. Provides nursing care on an intermittent basis to patients in their place of residence under the supervision of a Registered Nurse.

Education, License & Cert:

Graduate of an accredited school of Practical Nursing. LPN currently licensed in Pennsylvania. Knowledge of nursing practice at a level normally associated with a Practical Nurse. High School Diploma or Equivalent Required.

Experience:

Minimum of one year’s experience in the healthcare field. Appropriate business-related education and/or experience desirable, demonstrated skill and experience in computer hardware and software usage.

Essential Functions:

1. Data entry of clinical record from admission to discharge, including the submission of NOE, NOTR within 5 days, submission of HIS and CAHPS in a timely fashion. Data entry of hospice staff and volunteer timesheets.

2. Timely preparation of recertification and death packets.

3. Coding patient related invoices, obtaining approval signature and forwarding to finance office.

4. Maintaining the Hospice Patient Census and Level of Care Records.

5. Preparing end of month billing reports utilizing admission, discharge and census data.

6. Creating format of clinical forms, printing and tracking veral orders and POC’s, general typing and filing.

7. Providing unbiased and professional customer service to both internal & external hospice customers. Providing nursing care in accordance with the standards of the Nurse Practice.

8. Preparing the Medical Records for billing after completion of chart audits.

9. Coordinate tracking of physician signatures and paperwork.

10. Preparation of weekly IDG list and associated paperwork.

Other Duties:

2. Demonstrate sound nursing judgment, accountability and assessment skills.

3. Travel may be required.

Pay range for this position is from $21.80-$34.02
