CASE MANAGER II
FLSA Status:Full-time, Salary, Exempt
FTE:1.0
Salary Range:$55,000 - $60,000/annually

This position is responsible to assist individuals who have serious and persistent mental illnesses and/or chemical dependency to live self-directed and satisfying lives by providing services directly, minimally using outside resources and diverting hospitalizations by finding alternative interventions whenever possible. When there are other providers, there is close coordination and collaboration with these other outside resources to ensure the best possible service.
