CASE MANAGER II

FLSA Status:Full-time, Salary, Exempt

$55,000 - $60,000/annually

This position is responsible to assist individuals who have serious and persistent mental illnesses and/or chemical dependency to live self-directed and satisfying lives by providing services directly, minimally using outside resources and diverting hospitalizations by finding alternative interventions whenever possible. When there are other providers, there is close coordination and collaboration with these other outside resources to ensure the best possible service.

Primary Responsibilities

  • Establish rapport and maintain professionalism with individuals served.
  • Assist clients in identifying needs and aspirations to develop recovery goals and interventions for overall wellness.
  • Provide coaching on independent living skills such as financial, healthy lifestyle, vocation, education, housing, physical, and social skills.
  • Provide crisis assessment and intervention to promote in-home and in-community safety among individuals served.
  • Navigate and access available community and housing resources for individuals served.
  • Establish and maintain positive working relationships with other staff members, community service providers, and families.
  • Recognize and identify changes in mental health status to actively support individuals served in stabilizing their mental health.
  • Promote an inclusive, diverse, equitable, and respectful work environment.

Qualifications

  • Has a master’s or other graduate degree in behavioral sciences or related fields OR
  • Has a bachelor’s degree in one of the behavioral sciences or related fields and has 2,000 hours of experience in the delivery of services to persons with mental illness OR
  • Has 4,000 hours of experience in the delivery of services to persons with mental illness.

Preferred

  • One-year work experience in case management or providing community-based mental health services.

Additional Requirements

  • Must be ambulatory and able to navigate stairways.
  • A valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
  • The ability to lift and carry up to 50 pounds.
  • Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years.
  • Must be available to provide on call crisis support 24/7 in weekly intervals on rotation.
