Job Title: Case Manager
Full-time
Wages $19.00 - $23.00 (Depending on experience)
Location Murray, UT
GCS Foundation, Inc. is actively recruiting a full-time bachelor level mental health case manager to assist individuals with severe mental illness. Strong preference will be given to candidates with SSW licensure or ability to obtain licensure within 90 days.
Position Summary
GCS Financial Case Managers work as part of an interdisciplinary team to provide personal services, behavior management, skills development, and targeted case management services necessary for individuals with severe mental illness, to live successfully and independently in the community. This position requires skills necessary to establish and maintain budgets, pay client bills, and secure savings in case of emergencies. .
Required Qualifications
· Bachelor of Science Degree.
· Become a Medicaid Provider within first 90 days of employment (we assist you in obtaining this)
· Obtain case management certificate within first 90 days of employment (we assist you in obtaining this). Not required for individuals with SSW license
· Strong analytical and critical thinking skills
· Effective time-management skills
· Proactive and solution focused work style
· Detail oriented
· Customer service skills
· Ability to work in fast paced environment
· General knowledge of PC skills (w/MS Office)
· Reliable transportation and willingness to go out into the community when necessary
Preferred Experience
· Previous case management experience
· Familiarity with community resources
· Budgeting experience
· Familiarity with government agencies (Medicaid and Social Security Administration)
· Previous experience working with adults with severe mental illness
Essential Duties and Responsibilities
· Maintain active caseload
· Coordinate and advocate for care that ensures basic needs are met
· Collaborate care with outside behavioral health providers and family members
· Assist clients with establishing and managing budgets, paying bills and securing savings in case of emergencies
· Maintain client eligibility for Medicaid, Social Security benefits and other related programs.
· Maintain company standards for productivity andclinicaland clinical documentation .
· Protect the confidentiality of client information in accordance with HIPAA.
· I Adherence with companywith company policy and procedure.
· Perform other duties as assigned.
