Purpose of Position/Summary:

This position will serve as the Program Coordinator for the statewide Suicide and Overdose Fatality Review (SOFR) Program within the division of Fatality Review and Prevention at the Indiana Department of Health. The SOFR Program Coordinator will assist with meeting SOFR Program objectives by providing hands-on support and information to local SOFR teams and supporting suicide and overdose intervention and prevention initiatives at the state level. The Program Coordinator will assist with preparing case abstractions, facilitating virtual and in-person local team review meetings, and entering case data. The Program Coordinator will assist the Program Manager with conducting suicide and overdose prevention activities. The Program Coordinator will assist in the practical applications of SOFR efforts to increase local and state understanding of overdose and suicide fatality trends and opportunities for intervention and prevention. The Program Coordinator will ensure the timely entry of high-quality data, will promote and follow SOFR best practices, and will collaborate with SOFR Program staff to meet the needs of local SOFR teams and share their successes.

Essential Duties/Responsibilities:

  • Request and collect records for local and state-level fatality review teams, including medical records, death investigation records, Prescription Drug Monitoring Program records, educational records, and law enforcement records, on a weekly basis
  • Read and summarize case information in order to create slide decks for fatality review meetings on a weekly basis
  • Facilitate virtual and in-person local Suicide and Overdose Fatality Review Team meetings, in accordance with best practices, as applicable
  • Utilize Microsoft Excel to track and organize record requests and track case review progress
  • Enter case information into the national OFR database on a weekly basis
  • Communicate with local SOFR team coordinators about team successes, challenges, and needs
  • Provide local SOFR team coordinators with information necessary for review meetings
  • Assist in writing grant applications and reports for the SOFR Program
  • Establish high level collaborative relationships with local stakeholders that could include First Responder Agencies, Department of Child Services, county health departments, minority health coalitions, and other public and non-profit agencies and community partners.
  • Collaborate with FRP team members and participate in statewide reviews, when applicable.
  • Provide training and guidance to local SOFR teams to ensure best practice in case ascertainment, case review, and data collection

o Attend local review meetings

o Provide guidance on effective team development, including mandatory members

o Train local SOFR team coordinators in data collection and data entry into statewide data repository

o Ensure high level of confidentiality is maintained

o Anticipate and respond to SOFR team technical assistance needs

  • Assist the Program Manager in overseeing reporting requirements from local SOFR teams

o Maintain and update all records collected for case review, as necessary

o Assist local SOFR teams in case ascertainment and initiation, as necessary

o Maintain database of completed cases

o Communicate daily with local SOFR to obtain case information, as necessary

o Contact proper departments to obtain supplemental reports

  • Facilitate collaboration between local OFR teams and local strategic planning efforts.

o Act as a liaison between local SOFR teams and resources aimed at lessening the burden of overdose and injury/death in their community

o Maintain knowledge of up-to-date recommendations, resources

o Aggregate recommendations from local OFR teams and share with stakeholders and Program Manager at the state level

  • Communicate local SOFR team status and needs to Program Manager
  • Perform other duties as assigned.

Job Requirements:

  • Bachelor’s degree required
  • Knowledge of the organization and function of state and local health agencies
  • Ability to communicate complex public health theories and principles, both orally and in writing;
  • Ability to coordinate and maintain effective working relationships with other program stakeholders in both the private and public sectors.

Incumbent exercises extensive judgment and creativity to adapt agency’s goals and procedures. Incumbent must develop programs and policies to handle new or unusual situations that impact upon the program area and ultimately the entire agency.

Supervisory Responsibilities/Direct Reports:

Incumbent does not have any supervisory responsibilities or direct reports.

Difficulty of Work:

This position requires significant attention to detail and the ability to work with internal and external partners. The incumbent must also be able to resolve problems, logically develop ideas, interpret policy, and have superb oral and written skills.

Responsibility:

Incumbent has high level responsibility for ensuring agency compliance with Suicide and Overdose Fatality Review Program requirements. Reports to the Suicide and Overdose Fatality Review Program Manager.

Personal Work Relationships:

Incumbent works with department staff, legal counsel and public, state and federal representatives, and other stakeholders in the private sector for the purpose of developing and monitoring effective development of SOFR program and subsequent case review.

Physical Effort:

Position is primarily a desk job; however, statewide travel is required. Requires extensive computer use. The incumbent must be able to lift and transport boxes and case materials.

Working Conditions:

Incumbent works in a hybrid-format. Extensive in-state travel is required with some overnight travel possible.

Job Types: Full-time, Contract

Pay: $28.00 per hour
