Oversee and monitor the health, care and progress of clients receiving home health care.
Monitoring the health of our clients helps us provide them with the best home care possible.
- Assess clients' condition during every visit and track any observations
- Take and record measurements of vital signs such as blood pressure, temperature, heart rate, etc
- Administer medication as prescribed by the client's physician
- Dress or redress open wounds and assess progress of healing
- Continuously educate clients, their families and other caregivers on proper home health care strategies and procedures
- Provide palliative care as needed to keep the client comfortable
- Make recommendations for devices or tools that might improve the client's quality of life
- Listen to clients and be prepared to respond to concerns or requests
- Bachelor's degree in nursing; current nursing license in the state of NY
- Current CPR Certification; Access to reliable transportation
- Ability to work independently and with directions
- Compassionate and friendly demeanor
- Experience with Microsoft Excel, Outlook and Word to document and track client care activity
