Griffin Hospital is Hiring a Director, Planetree Program And Services Near Derby, CT

MAIN FUNCTION: Responsible for promoting and integrating the Planetree philosophy of patient-centered care and service excellence throughout Griffin Hospital. Coordinates and supports the delivery of Planetree Programmatic Elements and Amenity Services ensuring optimal performance of all services. Serves as a liaison between staff and administration related to enhancement of corporate culture. Plans and implements a comprehensive volunteer services program that provides for the screening, selection, recruitment, training and placement of volunteers to support the provision of hospital services and Planetree amenity services.
Supports the Planetree philosophy of health care provided at Griffin Hospital including the following:

Understands and promotes Griffin’s mission of providing personalized, humanistic health care in a healing environment.
Recognizes the patient as an individual that is a human being with different needs that must be enthusiastically met by all members of the Griffin Family.
Reviews HCAHPS monthly survey results and works with nursing unit and support services Department Directors and their staff to achieve performance and ratings improvement.
Actively participates as a member of the Person-Centered Care Council and other committees related to the implementation and delivery of the Planetree programmatic elements.
Is knowledgeable about the standards of the Planetree Certification Program. Ensures the hospital’s person centered care structures, processes, and outcomes meet the requirements for the hospital to meet the highest certification level.
Participates in performance improvement activities that measurably advance the provision of person-centered and safe care and that create an exceptional patient and family experience.
Through interaction with the Planetree staff and Planetree coordinators at other Planetree Network hospitals, is continuously aware of initiatives that could enhance the patient experience at Griffin Hospital and evaluates and recommends them for implementation at Griffin.
Reviews Planetree listserv queries and ensures that appropriate Griffin managers or Planetree programmatic coordinators respond.
Is a member of the faculty for the new employee orientation, Planetree retreats, and staff advanced retreats.

Organizes and facilitates monthly meetings of the hospital’s Patient-Family Advisory Council.
Identifies opportunities for organizational change and works collaboratively with other department leaders and staff to effectively drive organizational change.
Coordinates and supports delivery of Planetree Amenity Services including ensuring that Planetree amenity services staffed by volunteers are operating at established target levels.

In cooperation with the Patient-Centered Care Council establishes Planetree standards and measures that ensure the efficiency and effectiveness of Planetree initiatives and ensures that Planetree programmatic elements and amenity services provided and staffed by volunteers and others are operating at optimum levels.
Briefly visits new admissions daily (Monday-Friday) providing information about Planetree services including the Patient Information Packet and the Patient Care Conference and encourages them to obtain health information from the Community Health Resource Center and/or the satellite Centers.
Works with the Coordinators of the various Planetree Amenity Services to ensure that each program, including the Care Partner Program, is operating at the optimum level.
Meets monthly with the Coordinators of the various Planetree amenity services to assess performance levels and address issues.
Hosts an annual workshop with Planetree amenity service volunteers and other providers in appreciation of their service and to explore areas of improvement and establish annual goals.
Coordinates the Annual Planetree Day activities in concert with the Planetree Service Coordinators.
Works cooperatively with the Communications and Public Affairs Department to develop print and promotional materials that increase awareness of the Planetree Programs and Services.
Participates and assists with the coordination of Planetree site visits and tour lunches and ensures that the hospital is tour ready.
Attends monthly Management Conference and provides information about new or current Planetree services as appropriate.
Coordinates a meeting of the attendees at the annual Planetree conference and the development and implementation of an action plan to improve the patient experience.
Develops a Quarterly Report provided to the Operations Committee and the Person-Centered Care Council assessing the overall performance of the Planetree Program at Griffin Hospital and the performance of each of the Planetree Amenity Services with recommendations for improvement. The report will include the Planetree Report Card.
Oversees the Health Resource Center and the Information Desk.

Directs the operation of the Health Resource Center and the Information Desk.

Responsible for overall Lobby management.

Plans and implements a comprehensive volunteer services program that provides for the screening, selection, training, recruitment and placement of volunteers to support the provision of hospital services and Planetree amenity services. Develops and implements evaluation instruments for individual volunteers and the volunteer program.
Develops an annual volunteer staffing plan and a recruitment plan to ensure an optimal number of volunteers to meet hospital service and Planetree amenity service needs.

Develops and maintains a monthly vacancy report based on the annual volunteer staffing plan, Planetree report card, and training programs and materials for volunteers.
Maintains volunteer handbook to ensure current policies and procedures reflect role expectations.
Ensures attendance of new volunteers at hospital orientation program and develops and provides formalized department orientation program covering policies and procedures.
Plans and coordinates volunteer appreciation activities.
Attends community functions to recruit volunteers and increase awareness about the hospital and the volunteer program.
Provides for the maintenance of a volunteer information database system. Maintains records, monitors progress and the supervision of volunteers through direct observation and managerial feedback.
EDUCATION: Bachelor’s Degree
EXPERIENCE: Previous managerial experience working in a health care environment. Excellent communication skills. Experience with Microsoft Office and database management. Possesses leadership and teamwork qualities to coordinate wide-ranging volunteer and contracted resources to establish Griffin as the flagship hospital as a role model for the Planetree Network of member hospitals. Must be a strategic thinker and collaborative team player. Possesses personal qualities of empathy, respect, integrity, understanding and good judgment.
PHYSICAL DEMANDS/WORKING CONDITIONS: Must be able to assist patients with wheelchair service
