Responsibilities:

What You Will Do?

• Assists resident and/or family directly in adjustment to facility, staff, schedules, other residents, and changes in lifestyle.
• Mediates between residents, staff, and family members for conflict resolution.
• Completes an assessment and social history of each new resident within 7 days of admission.
• Records regular progress notes for each resident in accordance with facility policy.
• Supports Coordination of Overall Care Plan schedule. Updating schedule as required and notifying staff, residents, and family members of upcoming meetings. Coordinates family and/or resident participation in care plan meetings.

Qualifications:

What It Takes To Succeed?

• Bachelor’s degree in Social Work, Gerontology, Psychology, or other related field OR High School Diploma with completion of twenty hour State approved Social Services Course and three years’ experience in a long term care setting
• Demonstrable knowledge of the aging process and the special needs of the aging.
• Enjoy working in a fast paced, changing work environment.
• One year experience in a recreational program in a licensed health care setting preferred.
