Job Summary
PT/PD Hospice Social Worker
Greenwood Hospice, a not-for-profit hospice located in Lawrenceville is looking for a Part-time or Per Diem Social Worker to join our organization and serve our mission of providing outstanding hospice services in greater Mercer, Middlesex, Atlantic, Burlington, Somerset, and Hunterdon counties. Working up to 20 hours per week.
Qualifications
- Social Work required with current NJ LSW
- 1 year of supervised clinical experience; community health experience preferred, hospice experience a plus
- Excellent verbal and written communication skills
- Counseling skills with both individuals and groups
- Excellent interpersonal skills
- Knowledge of the process of death, dying and bereavement required
- Understanding of psychosocial factors operative in terminally ill patients and families required
- Unlimited access to a reliable car
- Experience with grief counseling
- Current state driver’s license and auto insurance
What you will be responsible for:
- Provide psychosocial assessments, direct casework, and counseling services to Hospice patients and their families.
- Participate in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished.
- Work collaboratively as a member of the Interdisciplinary Team.
- Prepare clinical and progress notes for the clinical record per Hospice, state and federal guidelines/standards. Prepare documentation for the clinical record per Hospice, state and federal guidelines and standards.
- Participate in taking on-call responsibilities as necessary including weekends and evenings.
- Maintain quality and productivity standards.
- Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
- Insure good community relations with current and potential referral sources
- Participate in business development activities as required.
- Assist in the development and presentation of programs to families, caregivers and others.
- Providing counseling and bereavement services as needed
- Assisting patients and families in obtaining needed community resources
