Position Summary
The Hepatitis C Care Coordination Nurse will provide care coordination for patients with hepatitis C infection who are in treatment for opioid use disorder at Greenwich House Center for Healing. The care coordination nurse will work with an interdisciplinary team comprised of on-site hepatitis C treatment providers, counselors, and a hepatitis C certified recovery peer advocate to engage patients in hepatitis C treatment and to educate patients about hepatitis C prevention, treatment, and harm reduction. Responsibilities also include engaging and supporting patients in treatment for hepatitis C, tracking patient progress in hepatitis C treatment, assisting the medical team with acquiring hepatitis C medications, providing and managing hepatitis A and B vaccinations, tracking cure rates and reinfections, and coordinating general medical care for patients engaged in hepatitis C treatment.
