Job summary
As a Family Link Worker (FLW) youwill be the link between the PCN and families with children under 5 registeredwith the GP practices in Greenwell PCN.
The outreach work of the FLW isessential in addressing the rising inequalities in health and social care andidentifying the unmet milestones in children of pre-school age.
The FLW willperform a variety of duties with the aim of improving the health and wellbeingof the children in families identified.

The bridging role between health, social care and thecommunity will facilitate improved access and supports the provision ofservices that meet the needs of identified children in the PCN.

Main duties of the job
Work with the rest of the early years years project team to look at the needs of families in the area and identify how to meet those needs.
Regular household visits to assess the health and social needs of the children under 5 years in the household, adopting a proactive and holistic approach when supporting the family.
To discuss and identify the familys needs and gain consent for participation in the Early Years pilot. Work.
Support families to understand the health and social care system and access the appropriate services for their needs.
Proactively signpost and refer into other existing community services and work with families and service providers to ensure that support is accessed.
Adopt a therapeutic approach to the needs of families building supportive relationships.
Keep records that reflect the familys needs.
Engage with the community to ensure health services are satisfactory and appropriate in their design and delivery.
Facilitate networks within communities to strengthen sources of informal support
Identify and advocate for the needs of individuals and the community by liaising between the health service and community.
Identify local community assets and promote a community-based prevention agenda.
Develop a strong working relationship with the Social Prescribing team.
About us
Greenwell Primary Care Network is an NHS Collaboration between 7 GP Practices in Greenford and Hanwell Oldfield Family Practice, Mansell Road Practice, Eastmead Avenue Surgery, Elmbank Surgery, Greenford Avenue Family Health Practice, Hanwell Health Centre and WestSeven GP.
We have a patient population of approximately 37000 patients, and currently have 7 staff members working across the network.
We have excellent relationships across the 7 practices and are working collaboratively to provide extended services to address any inequalities and to provide great care to our patients.
