Director of Social Work provides, as needed, direct social work service and counseling to each resident and his/her family in relation to:

  • Orientation of newly admitted patient to his/her new environment.
  • Emotional problems including assisting resident and his/her family with anxieties and stress precipitated by illness and disability; fears related to helplessness and death; acceptance of need for institutional and specialized care.
  • Financial problems including referral assistance with referral to Medicaid, assist with application and the yearly Medicaid re-certification.
  • Assists in addressing conflict of care issues and complaints
  • Assists in ensuring that concrete needs are met (eg. needed personal items such as clothing, T.V., phone, hearing aids, etc.).
  • Functions as a Residents Rights Advocate for all residents and their families.
  • Complete Psychosocial, Behavior, (F1, 2 & 3) and Mood (E1 2) sections of the MDS form on each resident within 14 days of his/her admission. MDS is done yearly or more often if change in patients condition. Complete Rap summaries when appropriate.
  • Administers mini-mental status exam as needed.
  • Writes a comprehensive psychosocial admission summary for the residents medical chart within (7) days of admission. This includes gathering significant information about his/her life which will lead to an understanding of the resident as an individual.
  • Administers a geriatric depression scale as needed.
  • Develop a plan of care and goals based upon the residents needs and update this in the medical chart quarterly or more often if needed.
  • Coordinates discharge planning. Maintains a close working relationship with other community agencies which supply supportive services to residents returning to the community (eg. VNA, Meals on Wheels, Waver Channeling, etc.)
  • Performs basic administrative tasks related to the job, i.e., maintains the residents files, answers the correspondence; keeps the patient information cards up to date.
  • Manages the activities of the social service department to accomplish the facility as well as the departmental objectives, makes recommendations on changes in policy, procedures, equipment and programs. This includes developing forms and policies and vprocedures. Reviews and updates these as necessary.
  • Functions as a active member of the Interdisciplinary Team by attending the resident care conference.
  • Interprets social, psychological, emotional and family problems to other team members to increase their understanding of the resident's attitude, behavior, and their reactions to illness loss and disabilities.
  • Invites resident and family members to attend care plan conference. Addresses problems and concerns of resident and family member at the meeting if they are not able to attend.
  • Development and maintain a working relationship with all disciplines and their personnel.
  • Participates in the orientation of new employees to assure that they are knowledgeable about the purpose and function of the social service department.
  • In conjunction with in-service training supervisor, present a yearly in-service program on the Patients Bill of Rights. Provides other in services to staff as needed.
  • Provide information to and assists resident and family members with developing written Advanced Directives.
  • Assist Nursing Department with completion of interagency upon resident discharge to community.
  • Maintains an ongoing quality assurance program for the social service department.
  • Services as a member of various committees as directed.
  • Keeps abreast of current social work issues as they relate to the elderly by reading and/or attending seminars.
  • Participates in activities of professional associations by attending meetings and serving on boards or committees.
  • Development and coordination of a family forum. Holds routine meetings and acts as the liaison between the facility and the family council.
  • Provides reports to the administrator in a timely fashion and as directed.
  • Complete Denial Letters (Medicare, Evercare, Bluechip, UHC)
  • Complete Medicaid Forms for Medicaid process (72.1 and 70.1 Forms)

 Director of Social Work Qualifications:

  • Bachelors Degree in social work from an accredited college or university.
  • Extensive knowledge of social work theory, methods, principles, and the application of various social work skills and techniques to individuals and groups.
  • Previous geriatric social work experience preferred.
  • Must have strong leadership skills.
  • Must have several years of social work experience in a nursing home or hospital.
  • Must be very knowledgeable about OBRA rules and regulations.
