• Resume (Minimum 2 years' experience)
• Skills checklist
• NURSYS report.
• BLS (AHA)
• 2 references (within last 1 year, at least 1 supervisor reference).
• Driver's license
• Covid Card
• Flu shot
• CO/Compact license only
