Your Goal:

This role is responsible for monitoring, assessing, evaluating, educating, and providing education and referrals to facilitate the coordination of medical health, oral health, and social services for low-income families. Additionally, the role would oversee the of the Family Case Manager staff.

Key Responsibilities:

• Provide daily management and oversight of Family Health Services support staff and operations.
• Deliver all forms of leadership, supervision, direction, training, and role modeling to all subordinate staff. Schedules staff appropriately, according to clinic needs.
• Coordinate with Human Resources department. in interviewing, hiring, and training new employees.
• Assist in development, interpretation and implementation of new policies and procedures.
• Ensure staff competency in their performance of family case management tasks and procedures.
• Accurately documents client activity in Cornerstone.
• Monitor and assess client/family needs.
• Provide Case Management in accordance with the standards of the Illinois Department of Human Services Family Case Management Program (IDHS FCM).
• Ensure compliance with Greater Family Health Policies and Procedures, Joint Commission Standards and all other applicable laws and regulations.
• Perform other duties as assigned.

Qualifications:

• Preference for a graduate of an accredited registered nursing program or licensed social work program (BSN or LSW degree), but a Bachelor of Science or Bachelor of Arts in a related field may be considered.
• Possession of Licensure in State of Illinois if applicable.
• Experience in management within a healthcare setting preferred.
• Knowledge of Joint Commission standards and healthcare compliance requirements preferred.
• Proficiency in database management and reporting.
• Bilingual in Spanish preferred, but not required.
