Greater Cincinnati Behavioral Health Services is Hiring an Outreach worker Near Cincinnati, OH
Join our mission to help individuals with mental illness and addictions to lead healthy and productive lives. GCBHS has a unique team that focuses on helping individuals who are experiencing homelessness and assesses them for behavioral health needs, our team is based out of the Men's Shelterhouse west of downtown Cincinnati.
These positions include a PATH Case Worker with expertise in mental health , and a PATH Case Worker with expertise in Substance Use Disorder, to serve adults who are experiencing severe mental illness, substance use disorder, and homelessness.

The ideal candidate will have social service experience and experience working with individuals experiencing homelessness.
Position Responsibilities :
- Strives to attain the goals as written in their job performance evaluations and abide by the standards as set forth in GCB's Compliance Plan.
- Screen individuals who are homeless for substance use disorders and mental health conditions.
- Makes multiple contacts as needed and provides basic needs items to engage homeless individuals.
- Interacts with agencies, service providers, and other community contacts to solicit and receive referrals.
- Develops psychosocial profile through interviews with individuals and information collected from previous service providers.
- Identifies and connects individuals served to services aimed at addressing mental health and / or substance use disorders, as well as other basic needs, including medical services, benefits, and housing.
- Attends staff meetings and training sessions as assigned.
Position Requirements :
- High School Diploma or Equivalent required
- The position requires frequent driving of the employee's personal vehicle. A valid driver's license, vehicle, and insurable driving record are required.
- Related experience working with persons experiencing homelessness, mental illness, and / or substance use disorder would be ideal.
- For Peer positions, individual must have previous personal experience with homelessness and / or mental health.
