Title: Behavioral Health Outreach Counselor/Therapist PT

Job Summary:
The Behavioral Health Outreach Counselor/Therapist will initially focus on administrative and outreach duties in support of GBMS’s community health, as they expand their client caseload. Gradually the position will transition to a primarily therapy role, with secondary role as outreach counselor. The Behavioral Health Outreach Counselor/Therapist will have assistance from and will collaborate with the Behavioral Health Services Coordinator and Behavioral Health Services Navigator.
This job description provides a summary of the major duties and responsibilities performed by individuals in this position. Incumbents may be asked to perform other tasks not specifically.
Primary Function and Responsibilities:
Develop and implement community outreach strategies.
Assist with administrative tasks and support clinic operations.
Conduct individual, group, and family therapy sessions, depending on need.
Collaborate with healthcare providers under our "Medical Home" model
Principal Duties and Responsibilities:
Reviews mental health referrals, and decides whether they are appropriate to the mental health program at Greater Baden.
Completes initial evaluation, and develops treatment plan as appropriate.
Submits information to state agencies as appropriate.
Provides individual and/or group therapy as appropriate to patients’ needs.
Maintains adequate records of therapeutic treatment plans, therapy sessions, and other contacts with patients and other providers.
Coordinates referrals to the psychiatrist, andassists the psychiatrist with patient care needs.
Conducts case management activities as necessary for clients.
Interacts with medical providers as needed for optimal patient care.
Completes encounter forms as appropriate ina timely fashion.
Complies with Joint Commission standards for ambulatory care.
Interacts with other agencies and community members as appropriate.
12. Submits information needed for credentialing files in a timely manner.
Participates in team/staff meetings; attends in-service meetings and applicable workshops.
Attends required meetings at Greater Baden in addition to team meetings and trainings.
Minimum Knowledge, Skills, and Abilities:
Master's degree in Counseling, Social Work, Psychology, or related field.
Licensure or eligibility for licensure as a therapist (LCSW, LMFT, LPC, or equivalent).
Previous outreach or community engagement experience preferred.
Strong interpersonal and organizational skills.
Commitment to ethical standards and confidentiality.
Fluency in Spanish is a plus, but not a requirement

Working Conditions:
Virtual position with opportunity for hybrid work, subject to client and department needs.
Will require driving between health centers for on-site mandatory meetings and outreach community events.
Assignments to particular sites, schedules and hours can be changed at any time with reasonable notice and may include weekend hours.Physical Requirements:
Prolonged sitting, primarily at a computer terminal
Occasional bending, stooping and stretching
Requires eye-hand coordination and manual dexterity sufficient to operate a computer, telephone, calculator, and other office equipment.
Requires normal range of hearing and eyesight to record, prepare, and communicate in writing, in person and by phone.
Require occasional lifting up to 25 pounds.
Language Skills:
Ability to read and interpret written or verbal documents and instructions. Ability to speak effectively to patients, employees and/or stakeholders of the organization.
Compliance:
This position requires compliance with Greater Baden Medical Services (Health Center’s) compliance standards, including its Standards of Conduct, Compliance Program, and policies and procedures. Such compliance will be an element considered as part of the Behavioral Health Outreach Counselor/Therapist’sregular performance evaluation.
Travel:
Must be able to travel between Greater Baden Medical Services, Inc. locations as required.
