Great Lakes Bay Health Centers is Hiring a Pediatric Care Coordinator Near Saginaw, MI

JOB SUMMARY

The Pediatric Care Coordinator is responsible for developing and implementing protocols and processes to improve developmental screening, linkage to services, and pediatric care across the organization. Develops, implements, an provides ongoing training to the clinical teams. Works closely with all clinical roles to develop best practices for screening and linkage to needed services. Responsibilities include collaboration with internal GLBHC departments as well as external service providers.

ESSENTIAL JOB DUTIES

  • Improve clinician efficiency by developing and/ or improving workflows and procedure for the ongoing improvement of developmental screening and pediatric care at GLBHC.
  • Serve in the capacity of a liaison between Early Childhood Development Programs in GLBHC’s service area in order to streamline the gaps that occur for referrals and completion of full evaluations.
  • Eliminate barriers to developmental screening assessments and well child care in primary care especially for marginalized populations (Black, Brown, low-socioeconomic status) by assessing and addressing social determinants of health (SDOH) and making services accessible at all GLBHC primary care sites. Leads, completes, and monitors outreach efforts to improve completion rates. Leads efforts to fully utilize available technology to advance the provision of well child care and developmental screening.
  • Collaborate with internal stakeholders and community partners to ensure a continuum of culturally appropriate education and care is provided.
  • Assess parents in developing a coordinated plan of care to assist them with navigating the health care system by providing culturally relevant and literacy appropriate education.
  • Provide educational resources for clinical staff and parents, ensuring compliance with performance standards and recognized best practices in the field of early childhood developmental assessments.
  • Actively participate in opportunities for continuous professional development by participating in Early Childhood meetings and trainings hosted in GLBHC’s service area.
  • Develop and implement clinical trainings for GLBHC clinical teams in collaboration with the clinical training team. Determine appropriate training needs to meet program goals and objectives. Actively participates in the orientation of new employees.
  • Provide guidance and oversight to GLBHC clinical teams through monitoring the use of the Ages and Stages Developmental Screening procedures and referral system.
  • Models appropriate skills/language in working with children and their families.
  • Develops, monitors, and participates in quality improvement activities and projects. Responsible to work with providers, support staff, and leadership to develop, maintain, and implement plans to achieve quality improvement goals and grant objectives. Participates in data collection as necessary to support grant reporting.

MARGINAL JOB DUTIES

  • Participates and/ or chairs various committees and department meetings as assigned.
  • Performs other Duties as assigned.

JOB SPECIFICATIONS

  • Education: Minimum requirement is completion of a medical assistant training program, licensed practical nursing program, or other healthcare related degrees or education may be considered in lieu of the specified training.
  • Licensure: Medical assistant certification or LPN licensure, if appropriate.

BLS required.

  • Experience: At least 2-5 years experience working in in a medical setting required. Pediatric experience preferred.
  • Skills: Ability to gather, organize, and perform data analysis assignments with limited supervision, preferred. Ability to work collaboratively with various departments in the GLBHC organization. Proficient in the use of Microsoft Office, especially Microsoft Excel, and common desktop/web applications. Ability to manage multiple projects and prioritize projects with short deadlines. Excellent verbal and written communication skills. Ability to be an effective team player with ability to relate well with others. Creative, resourceful, reliable, trustworthy, and discreet.
  • Personnel qualifications: Required emotional maturity, willingness to cooperate with the aims of the program, respect for children and adults, flexibility, patience. Culturally appropriate education or experience in collaborating with parents in the clinical environments of their children.
  • Physical Effort: Medium
  • Hours: Full time, flexible and varied
  • Travel: May require limited travel within service area. If use of personal vehicle is required, reimbursement for mileage provided at the existing rate of the corporation. Must be a licensed driver with proof of personal liability and property damage insurance.
