JOB SUMMARY
The HIV Pre-Exposure Prophylaxis (PrEP) Navigator works closely with the SIMS Nurse and STI/HIV prevention team to provide intensive care coordination, support and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care; performs related work as required.
ESSENTIAL JOB DUTIES
1. Work with STI/HIV prevention staff and others to identify potential PrEP clients and those in need of PrEP navigation services. 10%
2. Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care. Provide PrEP disease prevention and PrEP adherence education and counseling GLBHC. 10%
3. Assist PrEP clients with services such as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, management of clinical costs, appointment reminders, education on the benefits of PrEP and ongoing maintenance. 10%
4. Provide culturally sensitive services to people of diverse ethnic, economic, and educational backgrounds, as well as alcohol and other substance users, and people of different sexual orientations or gender identities. 10%
5. Complete screenings to identify readiness for PrEP and other barriers to care and work with client to identify position solutions. Assess benefits and insurance, and if needed, aid access payer programs. 10%
6. Effectively collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming. Serve as a fully functioning member of the STI/HIV prevention team, able to participate in daily activities and functions. 10%
7. Make referrals for external services needed to support PrEP care and follow up to ensure service delivery. 10%
8. Performs a client-centered approach and work to empower clients to navigate medical and social systems on their own. Provide outreach to relevant communities with a focus on men who have sex with men of color community. Establish working relationships with appropriate community resources in the area, including those providing mental health and social services to the target population. 10%
9. Provide ongoing follow up to ensure adherence to PrEP. 10%
10. Maintain appropriate patient files and timely documentation of services utilizing electronic prevention and/or medical records. 10%
MARGINAL JOB DUTIES
1. Participates in off-site public relations activities and community events as needed in collaboration with partnering agencies.
2. Attends GLBHC corporate meetings as required.
3. Performs other duties as assigned.
JOB SPECIFICATIONS
1. Education: High school diploma or equivalent. Associate degree in health or science related field preferred.
2. Licensure: Must possess and maintain a valid Michigan Driver's License. HIV Counseling, Testing, and Referral certification within one year.
3. Experience: One (1) year of experience working with HIV/STI prevention or care, community health programs, or human services arena.
4. Skills: Knowledge of community resources, PrEP/PEP/HIV & STI prevention education, community outreach and education, HIV prevention with LGBTQ community. Ability to work effectively with a diverse social, cultural, and ethnic populations in a respectful and sensitive manner, manage a variety of social media platforms, including Facebook, Twitter, and lnstagram, operate a computer and associated software, work effectively with officials of outside agencies, employees, and the general public, communicate effectively both oral and in writing. Comply with HIPAA regulations and client confidentiality standards
5. Interpersonal: Skills: Able to work with and communicate effectively with diverse populations. Ability to promote confidence, cooperation, and good working relationships among local/regional agencies, management, staff and departments. Pleasant and professional telephone manners. Able to communicate effectively with a diverse population. Self-starter with high initiative who can work independently or as a team.

7. Hours of Work: Full-time, flexible and varied. Evenings and weekends as required.
8. Travel: Local travel and minimal out-of-town travel may be a requirement of this job. Travel costs will be reimbursed at the existing rate of the corporation. Valid driver's license and insurance required for reimbursement.
