Overview

The Practice Performance Facilitator/Practice Transformation practitioner reports to the Practice Transformation Manager, CINQCare Inc., with accountability for aiding the practice transformation efforts for CinqCare Inc. The Practice Performance Facilitator should embody CINQCare’s core values, including Imagination, Partnership, Forthrightness, Humility, Courage, and Relentlessness.

Primary Responsibilities

The Practice Performance facilitator/ Practice Transformation practitioner will assist and support the practice in maximizing available information technology resources to achieve optimal patient outcomes and increased office efficiencies in areas such as:

  • Support improvement opportunities in partnership with IPA’s and other contractual partners in Value- Based payment agreements.
  • Serves as a liaison between CinqCARE ACO and IPA partners generating practice collaboration, accountability, and improvement.
  • Build trust and buy- in with key stakeholders and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives.
  • Conducts practice level assessments to identify areas of opportunities for improvement.
  • Analyzes data to ascertain root-causes and develops a plans of action to address practice level deficiencies.
  • Support Quality and Risk Adjustment improvement initiatives by assessing current state and working collaboratively with key stakeholders on population health efforts.
  • Displays strong time management and communication skills that align with CINQCARE’s core values.
  • Monitor, interpret, and track HEDIS and other quality performance measures, to optimize care outcomes and maximize revenue.
  • Drives success in population health performance by guiding practice teams to achieve best practice standards/benchmarks by providing ongoing provider engagement, training, and coaching.
  • Facilitate practice level meetings with practice champion to monitor, and track practice performance.
  • Monitor and prioritize key performance metrics throughout the transformation process.
  • Coordinates, as appropriate, practice interactions and/or follow-up with other CINQ Care functional areas. Works cross functionally with Network Management/Provider Relations, Medical Economics, and Data Analytics to optimize success.
  • Possesses a working knowledge of: internal enterprise-wide contractual areas of focus,. Medicaid VBP and Medicare entities to monitor and report metrics such as utilization trends, patient satisfaction, and other key performance indicators.
  • Collaborate with health plans to resolve operational and roster and panel management issues.
  • Perform PDSA cycles to implement change, solve problems, and continuously improve processes.
  • Supports the clinic by conducting family member outreach and engagement calls to facilitate access to care, close care gaps, meet HEDIS Measures, assist the clinical team in scheduling transition of care, and coordinate risk adjustment visits.

  • Additional responsibilities if a licensed nurse is in the role: The nurse has a dotted line reporting for clinical portions of the role to the Director of Clinical Operations
  • Daily, Weekly review of hospital/SNF individual patient medical records determining reason for admission, review of medical history, anticipating length of stay and discharge needs.
  • Family member Outreach. Conduct follow up phone calls to assist family members with scheduling primary care and post discharge appointments.
  • Conduct follow-up phone calls to ensure that family members receive primary follow up care as needed throughout the 7–14-day transitions of care timeline.
  • Review and analyze the patient’s treatment plan and ensure that it is both effective, cost-efficient, and aligned with evidence-based medicine guidelines.
  • Provide continuous review concurrently during the hospitalization and post discharge review by attaining the discharge summary to establish a personalized treatment plan
  • Patient action planning and goal setting to improve cost, quality, and patient experience
  • Interface with patients, physicians, hospital discharge planners regarding the status of patient and recommend services that support discharge care.
  • Assist in discharge planning, helping patients transition from inpatient to at-home care
  • Coordinate patient care by directing to the most appropriate specialist or ancillary provider
  • Participate in Daily Hospital Census Rounds forum to discuss the patient’s inpatient status, anticipated discharge and create a post discharge care plan in conjunction with the PHN, Nurse Case Manager and PCP, leveraging others in the Care Team as required
  • Work with our medical providers, consulting the primary care provider for input, and refer the patient to that provider in real time whenever appropriate
  • Offer patient education when appropriate.
  • Other duties as assigned.

Qualifications:

Education/Experience:

AAS degree or BA/BS, or related field and/or clinical license (RN). Experience with analytical skills and data analysis plus experience is HEDIS/STARS at the practice level. Coursework or experience in Quality Improvement, and/or facilitation skills preferred. Qualified experience in healthcare settings preferred. The ideal candidate will have experience in primary care practice performing practice transformation efforts.

Computer Knowledge:

Excellent computer skills required particularly related to Microsoft applications including Word. Excel, PowerPoint, and Outlook. Willingness required to explore and learn new or unfamiliar applications and databases, and when prepared to teach practices and team family members how to maximize technology Knowledge of statistical software and other analytic software preferred. Microsoft suite of applications with exemplary Excel/spreadsheet skills

Qualifications

  • Bilingual Spanish, Haitian Creole.

  • Collaboration: The Practice Performance Facilitator/ Practice Transformation Nurse will work closely with other business divisions to learn their needs, internalize their knowledge, and assist with solutions to achieve the business objectives of CINQ Care Inc.

Location: Practice onsite visits, as needed.

PHYSICAL DEMANDS

Requires sitting and standing associated with a normal office environment. Manual dexterity

needed using a computer keyboard. While performing duties of this position, the employee is regularly required to talk or listen. The employee needs to be able to see and frequently is required to use hands or finger, handle, or feel objects. The employee is frequently required to stand, walk, sit; and reach with hands and arms. Ability to travel frequently by car and/or public transportation.
