Overview:
We are seeking a Licensed LVN nurse to join our healthcare team at Granbury Rehab & Nursing (aka Granbury Villa). As a Licensed LVN, you will play a vital role in providing quality care to our patients. This position requires excellent clinical skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.
Duties:
- Perform nursing procedures under the supervision of a registered nurse or physician
- Administer medications and treatments as prescribed
- Monitor and record patient vital signs and medical information
- Assist with specimen collection processing and laboratory testing
- Provide direct patient care, including wound care, catheterization, and medication administration
- Collaborate with the healthcare team to develop and implement individualized care plans
- Educate patients and their families on disease management and self-care techniques
- Assist with discharge planning and coordination of post-hospital care
- Maintain accurate and up-to-date documentation in electronic health record systems (e.g. Point Click Care)
- Adhere to infection control protocols and practice aseptic technique
Requirements:
- Valid Licensed Vocational Nurse (LVN) license in the state of Texas
- Knowledge of medical terminology, ICD-10 coding, and healthcare systems
- Experience working with individuals with developmental disabilities is preferred
- Familiarity with hospice care principles and practices is a plus
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong attention to detail and ability to multitask
