Licensed Vocational Nurse - Walk-In Clinic Float
This position provides direct patient care, medication administration and advice of patient care needs under the scope of practice designated by the California Board of Vocational Nursing and Psychiatric Technicians. Receive a $3,000 sign starting bonus! This vacancy is at our Arden location. If selected for the role, the shift will be Monday - Friday, 8 hours, with rotating Saturdays.
Duties and Responsibilities:
* Provide care for diverse population ranging from newborn to geriatric including patients that suffer from chronic illnesses
* Phone advice supported by recognized industry protocols approved by the Chief Medical Officer and compliant with clinic policy
* Procedures and patient care as ordered, including venipuncture, wound care, port flushes, etc.
* Medication administration as ordered by qualified providers
* Coordination of post-hospital follow-up for primary care patients
* Results review including lab and imaging results for issues ranging from chronic diseases to sexual health
* Active support of health team members for the continuity of care provided to all patients at One Community Health
* Chart review, update and care coordination for assigned patients in community-based programs
* Convey critical assessment data and observations in a professional manner.
* Document all pertinent professional interactions.
* Complete all required documentation comprehensively and in a timely manner.
* Identify conflicts and seek consultation as needed for professional resolution.
* Interact in a collaborative atmosphere with providers, clinic staff and laboratory staff.
* Participate in quality improvement activities including annual training in quality improvement.
* This position functions as a member of the One Community Health team and as such actively collaborates with other clinical and non-clinical staff to ensure care is proactive and coordinated. This collaboration may include attendance at daily team huddles, care conferences and trainings. There will be ongoing training as appropriate. This position is responsible for reporting suspicion of child, adult (including spouses), elder, and dependent adult abuse as outlined in Penal Code, section 11166, Section 15630, and Section 1160.
Education/Experience Required:
* 1 year LVN experience requires
* Current active license to practice as an LVN in the state of California.
* Active CPR, BLS Certification.
* MA/EMT experience preferred
