About the Program

At Graham Windham our Family Foster Care programs help children toget back to their families, adopted into a new family or connected to someonewho is willing to be an anchor for children, throughout their childhood andinto adulthood. To help us do that, we have hundreds of loving & highlyqualified foster parents primarily in Brooklyn, Manhattan, the Bronx andWestchester County- who serve as our foster children's substitute family.The Treatment Family Foster Care (TFFC)program extends this support along with additional services to children andyouth with specialized emotional, mental health and/or other developmentalneeds.

About the Position

Graham Windham's Foster Care program seeks a Case Aide to Support the unit’s team in efforts to ensureexcellent quality care and healing to Graham Windham’s children; and ensurethat every one of our children is living in a nurturing, safe, and permanentfamily as quickly as possible.

Duties& Responsibilities (but not limited to):

  • Managephone messages, handling correspondence, ordering supplies, and performingother essential day to day activities, which include but are not limited to maintainingsign in/ out sheet, updating program’s office calendar, tracking staffschedules, establish and maintain an administrative filing system within theoffice, oversee records management, including off-site storage, create andmaintain case records, process new referrals and create intake packets.
  • Gathersand enters data in the New York State Office of Children and Family Services’CONNECTIONS electronic case record system (that data spans - progress notes,sibling visit grids & diligent search packages)
  • Managesmail and correspondence including family planning letters, filing &calendars.
  • Assists with processing applications for stateidentification, birth certificates, and social security cards for children andyouth in foster care.
  • Monitorthe submission of and follow-up regarding time sheets, purchase orders, checkrequests and other correspondences to other agency departments.
  • Visits foster families, youth, and caregiverhomes as needed.
  • Supervises family visits as needed.
  • Drives staff and clients (Foster children, parents, and fosterparents). Travels in and out of state when needed. Supports all functions thatattain and maintain accreditation and compliance with

all regulatory agencies, including NYC Children’s Services(NYCCS), the NYS Office of Children and Family Services (OCFS) and the Councilon Accreditation (CoA).

  • Provides/oversees proper record keeping and documentation for allactivities.
  • Collects, organizes and files report cards, attendance records,individualized education plans (IEP) and other data related to youth’seducational outcomes.
  • Manages and organizes case records for the unit – this includescreating new case records, closing old case records and maintaining separatedsibling logs.

Qualifications

Requires High School Diploma; Min. one (1) yearof experience working directly with families and children; Valid NYS Driver’sLicense & clean driving record; Willingness and ability to drive a car orvan within the 5 boroughs of New York City, Long Island, and counties North ofthe city. Bilingual in Spanish preferred; time management and organizationalskills; Computer literate, MS Office; Ability to work with diverse populations;Team oriented; Overtime when necessary.

An applicant for employment will not be hired if his or herdriving record reflects any of the following infractions; some examples includebut are not limited to:

  • Suspended, revoked or otherwise invalid license
  • Reckless , high risk or otherwise unsafe drivingbehavior within the past 3 years
  • 6 or more points on license
