GENERAL STATEMENT OF JOB
Under general supervision, provides nursing services and health care to Grafton clients. Performs a variety of nursing duties associated with monitoring clients health and general well being, managing health issues and providing health care. May perform work at a variety of Grafton locations. Reports to the Nurse Manager or Administrator.

MINIMUM TRAINING AND EXPERIENCE
Graduation from an accredited nursing program and 1 - 2 years of experience providing nursing services; or any equivalent combination of training and experience which provides the required knowledge, skills and abilities. Must have valid state license as an LPN or RN.
SPECIAL REQUIREMENTS
Licensure as a Licensed Practical Nurse or Registered Nurse; and a valid driver’s license with acceptable driving record.
