The Manager of Population Health Operations leads a team of care coordination staff to ensure optimal care for patients, including those with complex medical conditions, patients being discharged from the hospital and patients needing preventive screenings.The manager maintains clinical standards of care based on care management workflows aligned with an integrated, cross disciplinary team-based approach.The manager collaborates with administrators, medical staff, and other disciplines to ensure alignment with evidence-based population health management practices (NCQA PCMH and Health Equity Accreditations).
·Participates in certification, accreditation, and audit procedures to support optimal performance of the team.
·Collaborates and maintains effective relationships and liaises with leaders across Grady Health System, community partners, managed care organizations, and medical staff in pursuit of continuously improving patient outcomes.
·Effectively represent the department in meetings, conferences and other activities related to population health care management operations and related domains as assigned.
·Maintains an organized approach to work and the ability to function at a high degree of independence, urgency, accuracy, thoroughness, and attention to detail.
Qualifications:
·Active Nursing license in the state of GA required.
·Bachelor’s degree in nursing preferred with 5 years of clinical experience in ambulatory care management, progressive leadership preferred.
·Experience supporting NCQA accreditation and Certification in case management (CCM).
·Experience and knowledge of state and federal healthcare regulations, including reimbursement and coverage policies.
·Excellent written and verbal communication skills, including ability to present to large or small groups.
·Ability to work in a team environment and manage multiple deadlines and prioritize assignments.
·Well organized with a high degree of accuracy and attention to detail.
