Mental Health Case Manager
Reports To:
Director
Compliance Officer
Position Summary:
A Mental Health Case Manager based services to assist in collaborating closely with clients to develop treatment plans to improve their well- being and to identify their mental health conditions, such as psychiatric illness, substance abuse issues or other personal challenges. Mental health case manager conducts thorough assessments of clients to identify psychological needs, triggers and strengths and use these results to build informed and effective Individual Service Plans (ISP) to help clients improve their overall quality of life and reintegrate into society. Mental health case manager will render services in accordance with DHH/LDH and the agency’s policies, in a professional, culturally competent, and person-centered manner.
Qualifications:
1. Bachelor's degree or advanced degree from an accredited college or university in psychology, social work, or a related field preferred
or
2. Must have a High School Diploma or GED with a minimum of 2 years of relevant experience in related field preferred.
Responsibilities:
· Advanced knowledge of mental health conditions.
· Evaluating the goals and needs of clients to develop targeted treatment plans or support groups for those undergoing mental health conditions like substance abuse, mental illness and mental health conditions.
· Collaborating one-on-one with clients to help them develop coping mechanisms and set and achieve long-term goals for their mental well-being.
· Conducting in-depth assessments of client’s mental health states to create informed and effective treatment plans.
· Adding notes to clients’ files and records based on evaluations and treatment progress.
· Creating personalized Individual Service Plans designed to overcome their mental health and goals.
· Remaining calm and rational when resolving certain crisis client situations that can occur unexpectedly during treatments or examinations.
· Building after-care treatment plans and communicating them to clients for use once they are released from treatments.
· Performing home visits or contacting released clients to evaluate their progress and ensure they’re functioning on their own.
· Helping clients with their day-to-day challenges.
· Collaborating with other affiliated agencies to provide effective treatment and care to clients.
· Interpersonal and active listening skills.
· Problem-solving and critical thinking abilities.
· Teamwork and collaboration.
· Time management and flexibility.
· Organizational structure/skills.
Job Type: Full-time
Pay: $15.00 - $18.00 per hour
