JOB SUMMARY:
The primary responsibility of the Referral Coordinator working under the oversight of the Practice Manager is to promote continuity of care, linkage to care, care coordination, and provision of appropriate healthcare services. The referral coordinator will assist with improving quality standards. This position will help patients get connected to needed services and resources and thereby help improve health and reduce healthcare cost by better care management, focusing on care in the appropriate settings. The referral coordinator will ensure that referrals are addressed in a timely manner, maintain tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
The referral coordinator will assemble information concerning patients and referral needs. Per referral guidelines, provide appropriate clinical information to specialists. Be the system navigator and point of contact for patients and families. Identify and utilize community resources. Establish and maintain relationships with identified service providers. Telephonic scheduling of screening tests, educating patients about treatments. Ensure complete and accurate registration, including patient demographic and current insurance information. Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians. Assist patients with potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance). Remind patients of scheduled appointments via mail or phone.
