Direct Home Care Worker
Join our team at a growing Home Health Care Organization nestled in the heart of Michigan! This thriving patient-focused facility is seeking a dedicated Direct Home Care Worker to enhance our patient care team.
Responsibilities :
- Under supervision, provides supportive services to participants in various settings.
- Follows individual Care Plans for each participant.
- Assists in maintaining physical and emotional comfort and promoting independence.
- Performs a range of tasks including personal care, meal assistance, medication reminders, and transportation support.
- Ensures accurate and timely documentation of care provided.
- Responds to emergencies and reports deviations promptly.
Qualifications :
- High School Diploma or G.E.D. required, with a minimum of 2 years relevant experience or completion of a formal training program.
- Participates in annual in-service training and screenings.
- Demonstrates interpersonal skills and respect for participants and colleagues.
- Flexible scheduling ability and sensitivity to diverse backgrounds.
- Must meet standardized competencies and maintain medical clearance for direct participant contact.
Working Conditions :
- Requires driving within designated area with reliable transportation and valid driver's license.
- Variable working conditions with potential exposure to communicable diseases and chemicals.
