Gotham Companies is seeking a UAS Nurse with strong assessment and communication skills, as well as a deep understanding of various health conditions, disabilities, and community support services. We are looking for a dedicated, compassionate and person-centered nurse to help individuals thrive in their own homes and communities.

UAS Nurse Responsibilities:

• Perform UAS Reassessments in client's home or facility according to state guidelines.
• Document any concerns, conflicting information, other issues that surface during the evaluation process.
• Enter data into electronic UAS evaluation form.
• Assess eligibility in need of Long Term Care Services
• Ensuring compliance with regulatory requirements and healthcare industry standards
• Assisting in the coordination of interdisciplinary teams to ensure continuity and quality of patient care
• Providing education and guidance to patients and their families regarding healthcare resources and options available to them
• Reviewing patient medical records and assessing their clinical condition to determine the appropriate level of care

UAS Nurse Qualifications:

• UAS Certified (Uniform Assessment System for New York)
• Minimum 1 year preferred UAS / Home Care Experience
• RN License / New York State Nursing License
• Experience with QA tool (Lanavi) preferred
• Must be willing to conduct reassessment in a nursing home setting
• Ready to work and dedicated
• In person cases

Pay: $130 per reassessment visit (Fee for Service)

Pay: From $650.00 per week
