About the Position
The Goodwin Hospice team is seeking a full-time Hospice RN Float Nurse to support a team covering the Northern Virginia area within the beltway. Work with seasoned hospice professionals in caring for patients and families in their community and facility homes at the end of life. Candidates will support the hospice interdisciplinary group (IDG) by working with referral sources to determine clinical eligibility, conduct information visits and admit eligible patients at hospitals, home, or facilities. This position is primarily responsible for assisting in patient care delivery services to meet the care plan needs and goals of the patient and family.

Job Duties

  • Is assigned to make patient visits in coordination with the Clinical Supervisor that includes nursing assessment, planning, implementing, and evaluating all phases of nursing care.
  • Assesses patient needs and admission for service and follows protocol in admitting patients who meet eligibility criteria.
  • Assists the team in articulating continual hospice eligibility, including documenting appropriate subjective and objective criteria.
  • Helps to evaluate and make changes to the plan of care for home health aides and supervises their activities, including regular teaching.
  • Informs the Director of Clinical Services of unusual or potentially problematic resident/family issues.
  • Competent in evidence-based goals of care conversations with patients and families.
  • Participates in quality improvement reviews and evaluations of IDT activities.
  • Communicates with the attending physician, facility and family concerning patient/family care needs and response to interventions.
  • When patient is at a facility, understands the necessity of full partnership with the staff, including actively soliciting feedback, responding appropriately, and educating tactfully.
  • Initiates, implements, coordinates, and documents the patient’s plan of care with the IDT in accordance with nursing needs specified in the approved plan of care.
  • Delivers or supervises skilled nursing services using evidence-based knowledge regarding end-of-life care.
  • Documents all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service.
  • Reports changes in the patient’s medical or mental condition to the attending physician, facility staff and hospice team.
  • Teaches or supervises the teaching of household members, facility staff and volunteers who are responsible for assisting the patient with nursing or personal care needs.
  • Maintains regular communication with the Director of Clinical Services, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.

Job Requirements

  • A registered nurse who is currently licensed by the Virginia State Board of Nursing.
  • Minimum two (2) years’ experience as a professional nurse, preferably in a hospice, home care or critical care setting.
  • Certified Hospice and Palliative Care, preferred.
  • Proficient in Word, and able to master documentation software competently enough to assist others in its use.
  • Has a valid driver’s license and is able and willing to drive to patients’ homes to deliver care as part of caseload responsibilities.
  • Responds calmly and tactfully to stressful and emergency medical and psychosocial situations.
  • Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
