General Summary: Promotes independence by providing enriching community based activities, which allow for social skill building and maximum integration.

Principal Duties & Responsibilities:

  • Coordinate and facilitate community-based outings, activities and training for individuals and small groups.
  • Provides support to help individuals build social and life skills that complement their vocational goals.
  • Promote integration and natural supports in the overall community by planning and supervising activities that help remove or reduce behavioral and/or environmental barriers.
  • Facilitate participants’ development of meaningful relationships with individuals in the community based on similar interests / values.
  • Regularly communicates with the support team and others as defined by the individual support plan and as directed by Case Manager.
  • Maintains timely, well written and complete service documentation, including report of service hours, incident reporting, case noting and behavioral tracking of person served.
  • Provides personal care assistance as needed by person being served. Assists Care Manager in identifying and coordinating transportation for person served.
  • Assists in providing specific disability and related education to employers, co-workers and other community contacts of persons served.
  • Strives to improve knowledge and understanding of community relationships.
  • Applies time management skills to multi-task and is flexible to new situations.
  • Additional duties as assigned by Case Manager.

Description of physical working environment:

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Extensive walking and standing in daily work activities. Uses proper lifting techniques during work day.
  • Travel via auto and/or public transportation to various sites.
  • This role requires varying levels of personal care support for the individuals served.

Requirements

Knowledge, Skills & Abilities Required:

  • High school diploma or equivalent, some higher education preferred.
  • Knowledge of disabilities preferred.
  • Ability to handle challenging behavior, teach job tasks and coach adults with disabilities.
  • Familiarity with Microsoft Office.
  • Valid Wisconsin Driver’s License, proof of insurance and good driving record.
  • Access to auto for transport of person served, preferred.
  • Use of personal smart phone required. (Stipend amount based on FT/PT status )
