Job Summary:

Coordinates referral marketing efforts to drive admissions to Good Shepherd’s new, leading-edge rehabilitation hospital in Center Valley, Pennsylvania, from specified territories; develops and maintains relationships with physicians/providers/case managers and other referrers; understands individual and division admissions goals and strives to exceed monthly, quarterly and annual projections; evaluates patients to determine if they are appropriate for admission; provides education and information to referrers, patients and families; works with referral sources and the Good Shepherd Rehabilitation Hospital (GSRH) admission team to ensure smooth transitions.

Essential Functions:

DEVELOPS REFERRAL BASE

  • Develops and executes strategic referral marketing initiatives to increase admissions to the rehab hospital
  • .Tracks referral patterns and identifies opportunities to optimize referral capture
  • Develops plans to build new and grow existing referral relationships
  • Identifies potential new referral sources and understands the needs of systems, providers, and case managers
  • Executes referral strategies to meet individual, department, and corporate goals
  • Gathers, analyzes, and shares market intelligence
  • Connects referral sources with key internal stakeholders, such as Good Shepherd physicians, administrators and other providers, at in order to build trust and grow relationships.

PROVIDES EDUCATION

  • Identifies and meets with potential referral sources, patients and families.

Develops and presents educational in-services to potential referral sources including physicians, case managers, therapists, and insurance companies.Educates potential patients/families as to the rehabilitation program benefits and services of GSRH

  • Serves as a rehabilitation consultant, with attention to rehabilitation potential, therapy progress, discharge plan and feasibility and special equipment needs
  • Communicates with patient, family, case manager and admissions office regarding transportation, family housing, medications, equipment, and special needs
  • Provides potential patients and referral sources with the most up to date outcome information for GSRH and clearly articulates how GSRH differentiates itself against competitorsMAINTAINS REFERRAL TRACKING AND TREND ANALYSIS
  • Tracks visits/calls and accurately reports scorecard numbers and referral trends
  • Provides regular reports regarding outreach activities and business intelligence
  • Analyzes referral trends and reports on performance

IDENTIFY APPROPRIATE LEVEL OF CARE

  • Obtains referrals
  • Gathers all pertinent information that will assist in identifying if the potential patient is appropriate for post acute services
  • Collects all needed documentation and information for liaison to perform a thorough assessment, including ensuring the patient is medically stable at the time of transfer
  • Arrange for admission of patients

ASSURES ALL REQUIRED PRE-ADMISSION SCREENING DOCUMENTATION IS GATHERED AND REVIEWED

  • Determines that services are appropriate, reasonable and necessary.

Provides recommendations as to appropriate level of care and assist case managers with precertification

CONTRIBUTES TO TEAM EFFORT

  • Provides information necessary to facilitate referrals and subsequent admissions
  • Serves as a contact person for the post acute team, internal admissions team, clients and external sources to enhance outcomes

ONGOING CLINICAL EDUCATION

  • Maintains professional and technical knowledge by attending educational seminars, reading professional publications, establishing personal networks and participating in professional organizations

TRAVELS EXTENSIVELY AND WORKS INDEPENDENTLY

  • Works independently by traveling extensively to acute-care hospitals in an assigned territory to assess patients for potential admission, as well as building and maintaining professional relationships with all possible referring sources

OTHER DUTIES AS ASSIGNED

Qualifications:

Education:

  • Bachelor's Degree preferred

Work Experience:

  • 2-4 years of experience in acute care, or healthcare setting with direct clinical skills required
  • Prior experience in case management, marketing and/or a clinical liaison role preferred
  • Knowledge of acute rehab preferred
  • Previous experience working independently and the ability to make independent decisions required

Licenses / Certifications:

  • Driver's License required
  • Registered Nurse (RN), Registered Respiratory Therapist (RRT), Physical Therapist (PT), Speech Language Pathologist (SLP), Nurse Practitioner (NP), Physician Assistant (PA) or Occupational Therapist (OT), license required

Skills and Abilities:

  • Self-motivated; able to work independently with minimal supervision
  • Superb presentation/public speaking skills; must be effective in reaching and speaking with physicians, hospital case managers, discharge planners, etc.
