We strive to provide young people the necessary tools to realize their goals and set them on the path to living healthy and meaningful lives.Job DescriptionThe Good Seed is looking for a Lead Case Manager to join our dynamic team! The primary function of this position is client centered case management, support, engagement, and document facilitation. The ideal candidate will be a passionate quick-learner, who thrives in fast-paced work environments and possesses the ability to maneuver through challenges with a creative spirit. The Lead Case Manager will have general oversight and coordination of other case management. The Good Seed is a growing non-profit organization where there is growth potential. We are passionate and enthusiastic about the work that we do. If you have a can-do, roll-up your sleeves attitude, you will fit right in!Oversee all services and operational activities;Provide comprehensive case management that includes, but not limited to: screening and assessing clients; creating, implementing and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing and other community resources;Foster a professional and welcoming environment in all interactions with clients, guest, vendors and colleagues, regardless of differing identities, backgrounds, beliefs and choices;Manage and evaluate the performance of assigned staffed;Develop and maintain a professional and supportive relationships with staff and subordinates;Provide information regarding services and transportation as needed to people seeking services;Work in tandem with case managers to assist clients in reaching their case plan goals;Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed;Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with established guidelines;Actively participate in client- and program-related meetings and trainings;Adhere to strict boundaries and professional ethics in the care of others;Ensure that the facility is clean, free of potential hazards and safe at all timesEnforce house rules and policiesQualifications4-year degree in human services, or comparable combination of education/work related experience in a social service setting; working knowledge of case management and planning techniques;Experience working with homeless individuals, youth (age 16-25), and the continuum of care in a similar environment with the population of clients we serve; (Preferred)Detailed oriented and highly organized;Provide own transportation for work related travel as necessary;Maintain a clean driving record and valid driver’s license;CPR/First Aid CertifiedAvailable to work M-F, 8:00 a.m. to 4:00 p.m.Skills/abilityLeadership and managementMake sound decisions and has experience handling highly sensitive and confidential information;Multi-task in a fast-paced work environment;Quick to respond and work effectively in crisis situations, including the ability to de-escalate volatile situations and apply effective conflict resolution techniques;Proficient in with Microsoft Word and Excel;Excellent written and verbal communication skillsAttributesGenuine interest in helping youth;Patient, inquisitive, resourceful and flexible;Relatable to the young people we servePhysical AbilityMust be able to reach, bend, walk, stand and sit for long periods of time and climb stairs;Must be able to lift a minimum of 30 lbs.Full-time Salary/non-exempt, $36K-$40KAdditional InformationAll your information will be kept confidential according to EEO guidelines.
