About Golden Little Star:Golden Little Star is a leading community care organization committed to providing personalized and high-quality care services to individuals of all ages in Tarboro and the surrounding areas. Currently, we are proudly serving the communities of Edgecombe and Northampton Counties. As a participant in the Healthy Opportunities Pilot, we aim to integrate health and social services to address the root causes of health disparities and improve overall well-being.

Position: Community Care Worker - Location: Tarboro, NC

Responsibilities:As a Community Care Worker in the Healthy Opportunities Pilot program at Golden Little Star, you will play a crucial role in promoting health equity and community well-being. Your responsibilities will include, but are not limited to:

Collaborating with a diverse team of professionals, including healthcare providers and social workers, to support the holistic needs of clients.

Conducting assessments to identify social determinants of health affecting clients and developing care plans to address these factors.

Assisting clients in accessing health and social services, such as medical care, housing support, nutrition programs, and more.

Providing guidance and support to clients in navigating available resources and community programs, with a particular focus on Housing, Food and Nutrition, and IPV (Intimate Partner Violence) services.

Building strong relationships with clients and their families to foster trust and engagement in the program.

Advocating for clients and connecting them with relevant support networks.

Utilizing your experience in real estate and/or case management to better assist clients in housing-related matters.

Requirements:

High school diploma or equivalent.

Prior experience in caregiving, social services, public health, real estate, case management, or related fields is preferred.

Strong commitment to promoting health equity and social justice.

Excellent communication and interpersonal skills.

Ability to work collaboratively as part of an interdisciplinary team.

A valid driver's license and reliable transportation.

Flexibility to work various shifts, including weekends and holidays.
