Job Summary:

We are seeking a motivated and proactive LPN - Community Liaison and Outreach to join or start a care team. 

This role is crucial for driving new client admissions and fostering an ongoing high quality service by establishing and nurturing relationships within the community, including with physicians, local agencies, schools, and families of potential clients.

Key Responsibilities:

  • Create a service plan for the local care team including geographical area covered by the team, services to be provided, marketing plan for obtaining referrals and caregivers, and establish operating processes for the team to manage the care and the clients. 
  • A successful Community Liaison and Outreach will be able to grow the care team by at least 3 clients per month, you will be expected to be able to generate your own pipeline of clients as well as utilize the referrals coming into our platform from the payers. 
  • Develop and implement outreach strategies to promote our services within the community, resulting in a healthy pipeline of referrals and caregivers for the local team.
  • Establish and maintain relationships with local physicians, healthcare facilities, community organizations, and educational institutions in order to ensure that all local care needs are met and that care is coordinated.
  • Promote the care team’s services to potential clients and their families, providing them with detailed information about the care team’s services and conducting initial assessments to determine service eligibility.
  • Support the clients and caregivers in the process of converting a referral to an active client - creating a compelling client story for job ads, assisting in matching caregivers to the client, helping the back office to complete the authorization process by providing the supporting documents and ensuring a start of care is scheduled and performed in a timely manner. 
  • Guide potential clients through the intake process, ensuring a smooth transition to our services through communications, management of expectations and solving any issues that arise in the intake process.
  • Liaising between clients, families, and the home care team to ensure effective communication and service delivery through regular communication with the client and the care team and finding solutions to scheduling issues or other client satisfaction issues. 
  • Maintain accurate records of outreach activities and client interactions.
  • Ensure a reliable, high quality service by having a weekly touch point with the caregiver or client to coordinate the schedules and deal with any ad-hoc issues.
  • Assist in the hiring process of caregivers to the team, including interviewing, assisting in completion of the credentialing process, facilitating training and ensuring the caregivers are feeling confident about starting the care. 
  • Meet monthly with the care team to update the service plan and operating procedures, ensure a cohesive team culture and identify and solve any problems at the team level. 
  • Collaborate with the platform team and the back office to continuously improve the tools and the service available to the care teams. 

Required Skills and Qualifications:

  • Valid LPN license (PA) 
  • Proven experience in community outreach, sales, or a related field, preferably in the healthcare or home care industry.
  • Excellent interpersonal and communication skills, with the ability to engage effectively with diverse groups.
  • Strong organizational and time management skills.
  • Ability to work independently and as part of a team.
  • Must have a valid driver’s license and reliable transportation.
  • Knowledge of local healthcare systems and home care services is a plus.
  • Minimum of 2 years in community relations, client services, home care setting or a similar role in the healthcare sector.

Salary and Benefits

  • This position is hourly paid at $28/h. 
  • Uncapped commission paid as percentage of billable hours
