Position Summary:
The Advanced Nurse Practitioner is responsible for the health and wellness of the members who chooses to use the Living Well Clinic at the Glenridge on Palmer Ranch (GPR). The ANP is accountable for the provision and delivery of quality care in the Clinic. Provides personalized service and support that assures a high level of customer satisfaction for Members. Accountable for providing quality care in compliance with legal mandates and under the guidelines of the policies and procedures of The Glenridge on Palmer Ranch
Responsibilities:
1. Coordinates Member care services for all assigned programs in the Living Well Clinic.
2. Maintains a clinical record for every Member receiving services to ensure accurate documentation of treatments and billing information.
3. Assures that Nurse Practitioner services provided are consistent with the Nurse Practice Act
4. Provides Nurse Practitioner services in the Clinic or in the Member’s home as necessary.
5. Makes independent decisions concerning Nurse Practitioner services.
6. Obtains sputum, urine and other lab tests as indicated for diagnostic services.
7. Perform blood draws as indicated from the encounter or as ordered by Member’s Physician.
8. Consult with the Medical Director if Member condition changes or services required are out of scope of the Nurse Practice Act.
9. Assures that when a GPR Member is accepted for an assigned program of the Living Well Clinic, that there is a reasonable expectation that the services can be provided adequately and safely.
10. Assures that when services are to be terminated, the Member is notified of the date of termination and reason for termination; that the reason is documented in the clinical record; and that a plan is developed, or a referral made for any continuing care indicated.
11. Meets with Members, and/or family members, as necessary.
12. Participates in the implementation and maintenance of the infection control program for monitoring communicable and/or infectious diseases among the Members and staff.
13. Participates in the implementation and maintenance of the procedures for reporting hazardous conditions or equipment to the Director.
14. Assures that a stock level of medical supplies and equipment is always maintained on premises to adequately meet the needs of the Members. Makes recommendations for major equipment needs.
15. Implements operating procedures for all equipment used by assigned staff as necessary.
16. Ensure that supplies are used in an efficient manner to avoid waste.
17. Keeps abreast of economic conditions and recommends to the Director adjustments in services that ensure the continued ability to provide quality care.
18. Uses good judgment in determining needs of the company and in maintaining information delivery in conformance with recognized standards; and is expected to understand and follow all regulatory and facility policies
19. Keeps current on all changes in their area of responsibility, particularly regulatory changes as they relate to the clinic.
20. Uses good judgment in determining the needs of Members and working with the different departments and in maintaining delivery of those needs in accordance with recognized standards; and is expected to understand and follow all regulatory and facility policies
21. Maintains confidentiality of members’ PHI (Protected Health Information) and medical records information as well
22. Other various duties as assigned
Qualifications:
1. Must be a graduate of an accredited school of nursing with an advanced degree as an APRN, certification in Geriatrics preferred.
2. In addition, must hold a current valid RN license to practice in the State of Florida. At least 3 years of proven experience as an AP in a Clinic or long-term care setting.
3. Evidence of continuing education in area of the nursing process, and geriatrics care.
4. Must have thorough working knowledge of the responsibilities of the position and of regulations regarding professional practice in a clinic setting
5. Proficiency in understanding of computer software as it relates to the job. Experience with electronic medical records preferred
6. Excellent decision-making and analytical skills
7. Excellent communication skills and problem-solving skills
8. Excellent organizational and time management skills
9. Must be empathetic, energetic and have an affinity for working with a diverse senior population
10. Must be well organized and can work with limited direction, take initiative, and can plan and carry out responsibilities, organizing own schedule and duties to complete responsibilities
