POSITION SUMMARY
Responsible for providing individualized case management, student accountability and support services coordination for non-residential students.
o Follows all integrity guidelines and procedures and ensures no manipulation of student data.
o Assists students with personal and family needs and requirements to include transportation, housing, childcare, social services, etc.
o Conducts home visits and AWOL/UA retrieval.
o Monitors and provides guidance on career success skills and career transition activities.
o Conducts weekly non-resident student meetings/special program activities.
o Active member on case management and success teams to ensure student success in the program.
o Assists with Career Development and Individual Student Placement Plan development.
o Produces quality work/assignments in a thorough, timely and accurate manner.
o May transport students to and from Center and for appointments/activities.
o Monitors student progress and documents pertinent data in case notes and the ePCDP.
o Participates in CSSR/Assessment activities.
o Assists with CDSS programming to include Career Preparation, Career Development and Career Transition activities.
o Maintains appropriate personal attendance, accountability and work productivity standards.
o Ensures compliance with all aspects of the Non-Residential Program.
o Understands and applies job knowledge to effectively complete all required job responsibilities. Proactively maintains the skills required to perform job duties.
o Mentors, monitors and models the Career Success Standards as required by the PRH.
o Provides high-quality supervision and management for the student population and positively influences student behavior. Shows respect and courtesy to students and holds them accountable for their actions and behavior.
QUALIFICATIONS & EXPERIENCE
High School Diploma or equivalent required. A valid in-State Driver’s License required.
