ResponsibilitiesPerforms duties in alignment with the mission and policies of the organization. Performs duties in support of and in compliance with the performance improvement plan TJC and other licensing/accreditation and regulatory agencies. Embraces diversity by displaying sensitivity to different cultures and cultural backgrounds. Stroke Nurse provides: Direct patient physical assessment advice guidance education referral and follow-up for stroke center patients; serves as the patient's personal health assistant and is the primary contact for questions coordination of care and navigation through a stroke diagnosis; oversees quality measurements continuous quality improvement and all patient satisfaction initiatives on topics of Stroke; outreach to community groups partners and physicians on issues of stroke; program development and long term planning working with marketing and strategy as needed; The Stroke Nurse is expected to work closely with staff and physicians to create seamless coordination of care and be a key participant in service line development.
QualificationsPrefer critical care cath lab neurology experience
Experience in Patient Care Coordination preferred
Armenian speaking preferred
