SHIFTS:

3:00 p.m. – 11:00 p.m. (4 days on 2 off rotation)

OR

11:00 p.m. – 7:00 a.m. (4 days on 2 off rotation)

ESSENTIAL FUNCTIONS: Provides assistance to residents with ADL’S and IADL’S and assists in the maintenance of the facility environment. Participates actively in the provision of care services to the resident.

ADDITIONAL FUNCTIONS:

• Receives and gives report to charge nurse & team members at change of shift.
• Encourages or assists with activities of daily living (ADLs) such as bathing, grooming and with instrumental activities of daily living(IADL) such as shopping, housekeeping, laundry, socialization
• Practices effective communication techniques with residents, visitors, and coworkers and promotes a positive image of the facility
• Arranges, schedules, encourages resident activities, both spontaneous and scheduled
• Adheres to infection prevention and control practices
• Assists with admissions, transfers, and discharges of residents
• Prepares apartments for new admissions
• Transports and escorts residents
• Mentors new employees
• Documents care as necessary
• Participates in development, implementation, and evaluation of residents’ service plan
• Keeps environment, equipment, supplies, and resident furniture clean.
• Reports to supervisor:
  • Changes in residents’ status
  • Equipment and supply needs
  • Repair and maintenance needs
  • Safety hazards
  • Unusual occurrences or observations
  • Problems with completions of assignment
• Directs resident and visitor concerns to supervisor
• Responsible for obtaining required in-services
• Adheres to posted schedule. Submits requests for time-off per facility policy
• Responsible for adhering to all safety guidelines for protection of self, residents, and co-workers
• Works to promote maintenance of residents’ abilities at their highest level.
• Protects residents’ privacy
• Responds to emergency calls from residents

Other related projects, duties and responsibilities may be assigned as deemed necessary.
