We require someone who is purposeful and steadfast in their daily responsibilities. Successful Case Managers should meet the minimum qualifications:
Responsibilities:
    • Strong work ethic and a focus on patient care

    • Some night/weekend calls may be required based on the severity of the patients' and the care team's caseload
    • May have to occasionally step in for other care team roles

    • Superb communication and presentation skills
    • Energetic and outgoing
    • Proficiency in spreadsheet analysis using Excel
    • Proficiency with the MS office platform to include Word, Excel, Outlook, and PowerPoint, as well as other platforms
    • Proven ability to work independently and efficiently without close supervision
    • Willingness to actively engage in a collaborative team environment
    • Strong reading, writing and skills
    • Must be comfortable in an ever-changing environment
Requirements:
    • Update documentation for patients
    • HV documentation upload within 48 hours
    • Collaborate and manage a caseload of 150-200 patients per team
    • Provide necessary training to patients, home health aides, and nurses as required
    • Weekly calls
    • Conduct visits to patient's homes to build and strengthen relationships with the patient and their family members.
    • Conduct initial set up home visit for patients newly approved for home care and provide information on the program, as well as facilitate caregiver on boarding
    • Attend patient doctor appointments when needed, providing support, and advocating for suitable home care and DME
    • Some payroll assistance for contractors and Case Management
    • Compliance paperwork as needed from contractors
    • Report and document incident reports
    • Assist in the preparation and processing of patient assessments and care plans.
    • Coordinate with providers to secure approval for the patient's appropriate durable medical equipment (DME)
    • Collaborate with patients and their healthcare providers to oversee medical documentation for their home care renewals every six months

$19 - $21 an hour
