Giving Home Health Care is eager to find motivated and caring individuals to be advocates for our clients. In this role you will be a valued member of a care team, work with our clients to provide information about the program that we work with, and provide a personalized experience for each client.

We are looking for someone with great communication and time management skills to represent Giving by visiting patient home to ensuring the patients are receiving the best quality of care.

Responsibilities:

  • Visit patient homes and update patient care binders.
  • Ensure patients have the proper durable medical equipment in their homes. 
  • Collect and perform supervisory visit surveys. 
  • Effectively communicate and provide information to patients, caregivers, and physicians. 
  • Understand HIPAA and maintain confidentiality.
  • Display a warm and friendly attitude.
Minimum Qualifications:
  • Strong work ethic and writing skills.
  • Dependable transportation. 
  • Experience with MS Office products (Outlook, Word, Excel, etc.)
  • Minimum associate level education preferred. (High school diploma or equivalent is required.) 
  • 1 year of medical knowledge preferred.
