Job Overview

SFC requires a Georgia-licensed Registered Nurse (RN) to complete an on-site assessment of care recipients during the onboarding process and periodically through out their case.

We are looking for a motivated and flexible Field Assessment Nurse to join our team on a contract basis. Preference to Atlanta, Athens, Augusta, Macon, Savannah and Albany.

The pay structure for this job is a flat fee per assessment, plus mileage.

Responsibilities

• Cover your assigned service region, all within driving distance
• Within 1 business day of receiving an in-home assessment request, schedule the assessment to be completed within the next 3 business days
• Use your own car to drive to and from a member’s home to complete the in-home assessment
• Complete the web-based (accessible via smartphone) assessment tool
• Get the signatures of the care recipient and family caregivers on applicable forms
• Visually confirm that the family caregiver has installed the Givers mobile application, provide brief instructions and Q&A about their use of the app
• Provide training to the family caregiver for any tasks that they are assigned
• Perform other duties as assigned

Requirements

• Must hold an active Registered Nurse (RN) License domiciled in the state of Georgia
• Minimum two (2) years of recent experience in direct care of elders or adults with disabilities
• Ability to drive within assigned geographic area; valid driver’s license and automobile insurance is required
• Experience with Structured Family Caregiving or PSS/Attendant Care
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems)
• Additional state-specific qualifications may be required
