We are seeking a compassionate and experienced Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide skilled nursing care to patients in their homes, ensuring their safety, comfort, and well-being. Your primary responsibility will be to assess patient needs, develop care plans, and coordinate with other healthcare professionals to deliver high-quality care.

Job Details:

• $60/hr - $63/hr
• 5x8's
• 8:00am - 5:00pm
• 40 hours a week
• 13 week contract
• Start Date: ASAP

Requirements:

• Must have home health experience.
• Must have experience with OASIS Documentation.
• Weekened Requirement - 1 weekend every 4 weeks.
• Must have an active New Jersey license or compact license.

Duties:

- Conduct comprehensive patient assessments, including physical, emotional, and psychosocial aspects

- Develop individualized care plans based on patient needs and goals

- Administer medications and treatments as prescribed by physicians

- Monitor patient progress and make necessary adjustments to the care plan

- Provide education and support to patients and their families on managing their health conditions

- Perform case management duties, including coordinating referrals and scheduling appointments
