LPN with LTC Experience Needed!

Job Overview:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will provide direct patient care under the supervision of a registered nurse or physician. Your primary responsibility will be to ensure the well-being and comfort of our patients while adhering to all medical protocols and standards.

Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor patient vital signs and report any significant changes to the appropriate medical personnel
- Assist with patient assessments and develop care plans
- Provide wound care and dressing changes
- Assist with diagnostic tests and procedures
- Educate patients and their families on health conditions, medications, and self-care techniques
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records

Qualifications:
- Valid LPN license in [state]
- Proven experience in a healthcare setting, preferably in internal medicine or nursing home facilities
- Knowledge of medical terminology, procedures, and protocols
- Proficient in administering botulinum toxin treatment, gastric lavage, pain management techniques, and discharge planning
- Familiarity with health coaching principles and occupational health practices
- Strong communication skills with the ability to empathize with patients and their families
- Ability to work independently as well as part of a team
- Excellent attention to detail and organizational skills
- Basic computer skills for maintaining electronic medical records
