Description

POSITION SUMMARY

Patient Navigator as a member of the clinical care team serves as a source of contact for patients receiving services from Genesis Family Health. Patient Navigator enables physicians, nurses, support staff, and ancillary service providers to function as a collaborative care team centered on the needs of the patients. Patient Navigator will work to access additional services for the patient and assist with the navigation and referral to receiving services. Navigator position does not include direct delivery of medical, clinical, or other direct services which require licensure.

PRIMARY ACCOUNTABILITIES

Achieve Results

  • Assist patients to accurately complete assistance forms, including but not limited to internal UMMAM paperwork/documentation.
  • Provides accurate and timely evaluation of case plan; forwarding case information to appropriate internal providers.
  • Performs complete social needs evaluations on patients, including financial resources to assist patients in the navigation of internal payment plan as needed.
  • Educates patients of UMMAM patient portal, and sliding fee scale, etc. .
  • Provide updated prevention education information to patients.
  • Provides supervisor, provider, and other clinical staff with pertinent information on a frequent basis in regards to patient care. Using EMR for case management, and documentation.
  • Assist with translation and interpretation for patient appointments as needed.

Operational Excellence

  • Communicates to patients and external providers in a timely and accurate manner.
  • Maintain current patient instructional and health education materials for use with patients receiving care as deemed necessary in accordance to UMMAM referral services.
  • Maintain accurate and complete records of case management activities.
  • Maintain high standard of patient confidentiality.

Relationships

  • Coordinate services between UMMAM and local hospitals, clinics, or other related services being received; including but not limited to record of OB delivery information.

  • Coordinate services with in-region, and out-of-region agencies.

POSITION REQUIREMENTS

Education

  • Bachelor’s degree, preferably in, Human Services, Social Work or related field.

Experience

  • 3 or more years of experience with case management can be substituted for a degree.
  • Experience in outreach and inter-agency referral services.
  • Knowledge regarding clinical operations preferable
  • Must have strong working knowledge of computers, ability to learn office Electronic Medical Records software and related tools.

Physical/Environmental

  • Work may involve exposure to unusual elements such as extreme temperatures, unpleasant odor and/or loud noises.
  • Moderate physical activity. Requires handling average-weight objects up to 15-20 pounds.
  • While performing the duties of this job, the employee is regularly required to maintain sufficient visual acuity to read, and write.
  • Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
  • Will work with blood or blood-borne pathogens and will require OSHA training.
  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
  • Willingness to travel.

Additional Requirements

  • Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
  • Moderate physical activity. Requires handling average-weight objects up to 15-20 pounds, assisting with patients and standing and/or walking for most of the day.
  • Work involves considerable exposure to unusual elements such as extreme temperatures, unpleasant odor and/or loud noises.
  • While performing the duties of this job, the employee is regularly required to maintain sufficient visual acuity to read, write and operate equipment.
  • Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
  • All other duties as assigned
