Nurse Practitioner
Position Overview:
As a Nurse Practitioner specializing in AWV visits, you will be an integral part of our preventive care team. Your primary responsibility will be to perform comprehensive wellness assessments for residents in various SNFs throughout California. Through your expertise and dedication, you will help identify health risks, promote well-being, and create personalized care plans, ultimately contributing to the overall health and happiness of our residents.
Responsibilities:
Conduct thorough and empathetic Annual Wellness Visits (AWV) for residents in Skilled
Nursing Facilities.
Perform in-depth health assessments, including medical history review, medication
evaluation, cognitive function assessment, and fall risk analysis.
Screen for mental health concerns, providing compassionate support and appropriate
referrals as needed.
Collaborate closely with the interdisciplinary team, including physicians, nurses,
therapists, and other healthcare professionals, to develop individualized care plans
tailored to each resident's unique needs.
Educate patients, families, and facility staff on preventive care measures, healthy
lifestyle choices, and self-management techniques.
Ensure accurate and up-to-date documentation of all assessments, interventions, and
patient interactions.
Stay current with Medicare guidelines and regulations concerning Annual Wellness
Visits.
Requirements:
Valid and unrestricted Nurse Practitioner (NP) license in the state of California.
Board certification as an Adult-Gerontology or Family Nurse Practitioner.
Proven experience conducting comprehensive health assessments, preferably in
geriatric or SNF settings.
Familiarity with Medicare guidelines for Annual Wellness Visits.
Excellent communication skills, both verbal and written, to effectively engage with
patients and healthcare teams.
Empathetic and caring approach to patient care, fostering a trusting and supportive
environment.
Ability to work independently while collaborating effectively within a team-oriented
environment.

ethics.
