Position: Staff Nurse LPN – Health Clinic Hammond, IN

Regional Health Systems is Northwest Indiana’s largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through or community mental health center and federally qualified health center. Position Purpose:

To provide Health Care to patients in a clinic setting by interventions which are directed toward:

1. Health Promotion and Health Maintenance

2. Assessment, Treatment and Evaluation

3. Day to day patient care in a health clinic setting

4. Administer and monitoring pharmacologic treatment

5. Health teaching, crisis intervention, community resource and collaboration with other health care providers

Essential Function:

Requirements:

• Practical Nursing Degree
• Licensed Practical Nurse in the State of Indiana, no experience required – one year of experience preferred.

Shift Details: Full-time – 5-8hr shifts.
