SUMMARY OF POSITION:
The Case Coordinator position is a 1.0 FTE position working in two different .50FTE positions within Gateways Homeless Services Programs: (1) At Homeless Outpatient Services, the Case Coordinator is responsible for providing case management services, principally supportive of improvement in life skills and ADLs, to adults with serious mental illness. (2) At the Wellness Center, the Case Coordinator assists consumers with locating, obtaining housing, maintaining housing, and developing housing resources. Both positions require budget adherence, submission of reports, chart files, group activities and facilitation of oversight entities (including ACOF, HACoLA, LAHSA, DMH, Shelter Care Partnership.)
ESSENTIALS DUTIES:
  • Ability to work and relate effectively with multidisciplinary treatment team, clients and public
  • Ability to assess patients for dangerousness and intervene in a timely, calm, appropriate manner
  • Ability to communicate effectively in English, both verbally and in writing.
  • Ability to work independently, uses good judgement, and remains calm in crisis situations.
  • Knowledge of basic counseling skills. Understanding of individual, group, and family dynamics.
  • Ability to work indoors and outdoors (i.e. take clients on outings and transportation of supplies)
  • Ability to remain calm, objective, and dependable in situations where residents are emotional or uncooperative.
  • Computer skills in word processing for creating and typing reports or other related documents.
  • Crisis intervention for assessing and de-escalating assaultive behavior
EXPERIENCE/QUALIFICATIONS:
Minimum Experience Required
  • One (1) year experience mental health, counseling, or substance abuse treatment
  • Academic training in Behavioral Sciences
Desired Experience
  • Two (2) years' experience in mental health, counseling, or substance abuse treatment. Experience in homeless outreach, housing placement, LAHSA program or homeless services
EDUCATION & CERTIFICATES
  • Bachelors Degree in Human Services, Psychology, Social Work, Sociology, Criminal Justice, or related behavioral health science
    OR
    Bachelors Degree in Unrelated Area of Study AND Two (2) Years Experience in direct client mental health service
    OR
    Four (4) years in direct client mental health service
REQUIREMENTS
  • Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
  • Valid California Driver's license.
  • TB clearance.
  • Driving record acceptable for coverage by Gateways insurance carrier.
  • Management Of Aggressive Behavior*(M.O.A.B)
  • Fire and Safety Training*.
  • First Aid Training Certification*.
  • CPR Certification*.
  • Crisis Prevention Institute Training (CPI)**
* Note: If indicated certifications/licenses are lacking, they must be obtained within 180 days of hire.
**Training needs to be completed within 30 days from the date of hire on a designated date provided by on-boarding. Staff need to be certified at the commensurate level for their role to perform their job duties (staff will be taken off the schedule if certification is not achieved).
