Gateway to Learning is Hiring a Home-Based Services Specialist- Bilingual Spanish Near Chicago, IL

Mission

The mission of Gateway to Learning is to provide lifelong learning to adults with intellectual and developmental disabilities that promotes active inclusion at home, at work, and in the community.

Position

The Home-Based Services Specialist is responsible for providing Self-Directed Assistance for individuals and families who receive Home Based Waiver Services through Gateway to Learning and are funded by the Home-Based Waiver. As a Qualified Intellectual Disability Professional (QIDP/Q), the Home-Based Services Specialist ensures that the services received are consistent with the individual’s wishes, needs, and desires. Reporting to the Director of Home-Based Services, and supporting primarily Spanish speaking families, the Home-Based Services Bilingual Spanish Specialist is a salaried, full-time position.

Responsibilities

  • Collaborate on personal plans with individuals, families, Independent Service Coordinators, and other service providers.
  • Assist families and/or Employer of Record with completion of HBW required forms such as Medicaid Spenddown forms (HFS-2653), Implementation Strategy forms, and documentation notes to support billing.
  • Provide training to participants and families on monthly billing for the PSWs each month, timekeeping and other employer related tasks for PSWs, individual's skill-acquisition, prior approval requests.
  • Manage completion of Service Agreements for all waiver services.
  • Assist with managing the monthly budget.
  • Support the accurate submission of PSW timesheets to ACES$ and coordinating with providers and ACES$ as necessary.
  • Assist with communication and problem-solving strategies for PSWs hired directly by the individual/family.
  • Provide medical advocacy as needed.
  • With the Director of Home-Based Services, respond to after hours or emergency situations, medical or otherwise such as to meet needs of the individual.
  • Assist with end-of-life planning/care, pre-planning and arrangements for clients and/or loss of their family members as needed.
  • With the Director of Home-Based Services, submit billing for services to appropriate vendors in a timely manner. Meet billing deadlines and revenue goals.
  • Liaise and collaborate with SDA stakeholders including but not limited to: participants, family members, Independent Service Coordinators, guardians, and other service providers.
  • Serve as Qualified Intellectual Disability Professional (QIDP/Q) for participants in HBW division services.
  • Participant in GTL All Staff meetings and In-Services.
  • Collaborate with Gateway to Learning CDS and Employment Teams as needed.
  • Maintain training requirements including DHS Rule 50 and CPR; and as assigned.
  • Maintain professionalism with all stakeholders.
  • Serve as a role model for all Gateway to Learning Employees.
  • Conduct all other duties as assigned.

Qualifications

  • Proficient in oral and written Spanish.
  • Qualified Intellectual Disability Professional or eligible (BA required, in related field and 1 year working with persons with disabilities).
  • Managerial experience a plus
  • Familiarity with Home Based Waiver licensing and operational requirements a plus
  • Well organized
  • Ability to work with families.
  • Must be a “self-starter”, show initiative, and work independently.
  • Must be a “team player” and be able to work well with others.
  • Excellent communication skills.
