Demonstrates responsible decision making in planning, providing, and delegating care based on assessment.

Communicates effectively. Establishes and maintains two way communication with peers, staff, physicians and administration.

Follows through on problems that may compromise patient care by using the appropriate chain of command.

Communicates appropriate information to charge nurse/supervisor/manager/physician in a timely manner.

Documents accurately and timely according to guidelines and physician orders to maximize patient care.

Controls and accurately records the use of narcotics assigned to the department.

Maintains a clear, neat and safe environment for patients and staff.

Insures supplies and equipment for patient care are readily available and organized in an efficient manner. Ensures a FIFO (first in first out) inventory system, avoiding the possibility of sterile product expiration.

Correctly identifies and uses appropriately personal protective equipment.

Seeks information/guidance from others as needed.

Performs accurate patient identification requirement per department protocol for HIPAA compliance.

Fosters teamwork by offering assistance to others.

Demonstrates appropriate awareness for suspected abuse/neglect, and takes measures to report them according to protocol.

Takes call as required

Nursing Process

Organizes and prioritizes elements of patient care for self and others (LPN, PCT, etc.), to include patient hourly rounding and bed side report.

Delegates appropriately; tasks and duties to other members of the health care team.

Performs accurate ongoing assessment of patient status.

Effectively role models professional behavior.

Ensures continuity of professional care.

Demonstrates appropriate interpersonal skills for all age groups.

Utilizes, observes and documents restraints per physician order.

Responds to patient and family requests promptly.

Ensures each patient’s general hygiene needs are met daily, teaches patient/care giver techniques as appropriate.

Collects and labels specimens for diagnostic lab per hospital policy and ensures delivery to appropriate department.

Checks equipment prior to use for malfunctions and initiates repair request as well as cleans and returns equipment to storage area after use; keeping traffic areas clear.

Utilize supplies in a cost effective manner

Charting reflects patient problems identified in the plan of care and is timely and accurate.

Assist the director with managerial duties, staff education, and compliance.

Technical Expertise

Provides care to assigned patients including the activities of daily living, therapies and interventions.

Uses patient safety techniques in the delivery of medications.

Accurately calculates and administers all medication as prescribed, including drips in mg/min, mcg/kg/min, mcg/cc, and unit/hr as appropriate and accurately counts narcotics, signs for usage and waste appropriately.

Safely performs IV therapy.

Monitors/reports in a timely manner any adverse drug reactions.

Clinical competencies reviewed and completed.

Admits, discharges and assists in establishing a plan of care using the nursing process and a multi-disciplinary approach which incorporates the patient’s correct growth and development for all age groups.

Accurately follows policies in patient care including blood administration, specimen labeling, and patient identification.

Performance Improvement Activities

Accurately collects improvement performance data as directed.

Actively participates in the development and implementation of process improvements.

Frequently attends/participates in departmental staff meetings.
