Our Skilled Nursing Facility is seeking a Speech Language Pathologist (SLP) responsible for assessing residents requiring rehabilitation care
·The SLP will determine each person's medical condition, functional capabilities, and limitations.
·Establish and administer a treatment program based on the specific goals determined according to the patient's capacity and tolerance under the direction of the Rehab Director or Physician.
·Educate patients and families about the care plan.
·Must be Compliance with confidentiality & HIPAA
Qualifications
·2 years of experience
·Valid SLP license for the State
·Empathetic and Caring personality
·Graduated from an accredited Speech Therapy University
·Well-versed with EMR documentation, such as Rehab Optima
