PURPOSE:
  • The Case Manager / Housing & Employment Specialist (HES) is responsible for engaging clients to uncover their own options and solutions. 
  • The HES facilitates family stability planning, and provides assistance, support, and resources to client families in the FUSION program, helping them to build their support network and the necessary resiliency to housing stability and self-sufficiency. 
  • The HES collaborates with community partner agencies and property owners to ensure the family stability plan is achievable and that permanent housing and gainful employment are secured. 
  • This is a collaborative position that supports both FUSION’s transitional housing and emergency shelter clients.
  • This position is key to the success of clients becoming stable and breaking the cycle of homelessness and poverty. 
  • All FUSION employees are required to adhere to all State and Federal mandates including HIPAA regarding confidentiality and record retention.
RESPONSIBILITIES:
  • Provides compassionate and professional services utilizing client-centered and trauma informed approaches such as progressive engagement, motivational interviewing, and stages of change models of service delivery.
  • Engages clients in planning, monitoring, and evaluating their housing and employment stability and needs.
  • Creatively engages families in finding employment and permanent housing opportunities.
  • Serves a liaison between clients, providers, family members, property owners and other resources as identified by the clients.
  • Assist clients and their families in maximizing their abilities for self-determination by enabling them and empowering them in decision-making to the greatest extent possible.
  • Refers clients to employment development opportunities in the community including but not limited to; resume writing, interview skill-building, GED classes, ESL Classes, employment and internship opportunities, etc.
  • Provides flexible support to client families based on the unique needs of each family.
  • Consults with Program Manager, other HES, and Executive Director regarding any client related issues as appropriate.
  • Maintains accurate client files and information including case notes, family stability plans, progress reports, and resource agency correspondence.
  • Completes weekly, monthly, quarterly reports with outcome narrative, plus data reports on program performance as requested.
  • Responsible for HMIS data entry to ensure timely and accurate information is provided, either directly or through coordination with others.
  • Performs client orientation, initial assessments and family stability planning utilizing applicable tools within the first 72 hours (whenever possible) of clients entering the shelter.
  • Using a progressive engagement approach, meets weekly, or bi-monthly, with FUSION clients for 1:1 face-to-face session to review and revise the family stability plan and celebrate successes.
  • Communicates daily with FUSION staff and management and assists with shift coverage as necessary due to vacation, illness or emergencies.
  • Ensures clients adhere to all FUSION guidelines and protocol and reports any concerns to management.
  • Communicates with appropriate person regarding any facility or operational issues and/or concerns.
  • Contacts appropriate person immediately and provides written incident reporting when critical incidents occur such as fire evacuation, police intervention, 911/EMS, mental health, or medical emergency.
  • Objectively mediates conflict and disputes with clients while following all program protocols and facilitates grievance policies and procedures when applicable.
  • Provides screening and intake of incoming client families when necessary.
  • Must be willing to perform other duties as assigned and works well in a collaborative team environment.
IDEAL QUALIFICATIONS:
  • Bachelor’s degree from an accredited college or university and/or equivalent experience of 3 years of case management experience working with vulnerable populations.
  • Experience providing innovative best practices in case management including motivational interviewing, trauma-informed care, stability/housing planning, and progressive engagement models/tools.
  • Understands and adheres to state and federal confidentiality regulations including HIPAA.
  • Demonstrates the concepts of servant leadership, emotional maturity, empowerment, service, compassion, patience, grace, accountability and professionalism toward all peers, staff, volunteers, and clients.
  • Experience providing programming for diverse populations while being aware of, and sensitive to cultural diversity.
  • Supports and models the values and mission of FUSION to staff, volunteers, clients, and the community at large.
  • Must have the ability to cope well in a sometimes-stressful environment and not “buy into” the perceived crises of others.
  • Must have good literacy skills including the ability to gather, document, and report information clearly and to share effectively about FUSION and its programs.
  • Must have good computer/data entry/communication skills including the ability to collect, enter and report information correctly using tools such as Apricot, Microsoft Office and Teams.
  • Must sign an oath of confidentiality covering all staff, volunteer, and client-related information.
  • Must have a personal self-care routine that contributes to a professional and healthy work culture which supports compassion, accountability, open communication and respect.
  • Must have extensive knowledge of available community resources and a clear understanding of how to access these resources.
